On 03/01/2023 at 9:00AM Licensing Program Analyst (LPA) A. Curry and Licensing Program Manager (LPM) L. Dyson conducted an unannounced random inspection. LPA met with designated site principle, Sharon Travers. This is a preschool age program licensed for 42 children which operates Monday – Friday from 8:00 AM – 3:00 PM. Per the Director there are 65 children enrolled.

LPA A. Curry began facility tour. All areas identified on the facility sketch were inspected. LPA observed 48 children with 8 staff. Teacher-child ratios were observed to be in accordance with Title 5 regulations. The Licensee is not within the capacity specified on the license (Please see 809D). Staff confirmed 8 children were absent today. Staff names were recorded. All children were not observed to be under visual supervision of a teacher at all times (Please See 809D). In two separate classrooms a child went to the restroom and staff did not have visual supervision of the child. Site principle is aware that staff should have 100% visual supervision of children at all times.

LPA observed the facility to be clean, safe, sanitary and in good repair. Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and hand washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, an area in the classroom away from other children is used as an isolation area. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted.

The sink in the school age 1 classroom exceeded the acceptable amount of lead allowed in a child care facility (Please see 809D). Staff indicated the sink is occasionally used to refill the children's water bottles. The sink is to be placed out of use and made inaccessible to children until the sink has been repaired or permanently removed. Sink must be retested prior to being used to refill water bottles for children. Facility sketch, LIC 9275, and LIC 9276 forms have been requested and not received yet.

Snack menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. There is drinking water available in all indoor classrooms and individual drinking containers are taken outdoors. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. The licensee takes measures to keep the facility free of flies, other insects and rodents.

Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Outdoor activity space surface is maintained in a safe condition and is free of hazards. Areas around and/or under climbing equipment have cushioning material to absorb a fall. Head teacher Delores Jeff states there are no weapons or firearms on the premises.

Sign in and out sheets are electronic and were reviewed by LPA. Staff and children Records were reviewed. Verification of CPR/First Aid and health preventative practices documentation were reviewed.



This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies. Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Also, LPA A. Curry informed the facility representative to provide a copy of this licensing report dated 03/01/2023 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

The facility will immediately place the sink out of order and place a sign next to the sink indicating the sink is out of order. The sink will no longer be used until repaired or permanently removed. The sink must be retested and brought back into compliance before being used to refill water bottles for children.
By 03/31/2023 the facility will submit a written plan on how and when the facility will come back into compliance. ex. details of the repairs or permanent removal of the sink.
