25 | Licensing Program Analyst (LPA) Paul Petersen conducted an unannounced random annual site inspection for this Title 5 facility on 05/21/19 at 1005. LPA met with clerical assistant, Asia Capers, and toured all areas currently utilized by children in care for a health and safety inspection. There were 31 preschool age children in care along with five teaching staff (all five qualified teachers), two work study assistants, and one outside agency consultant. The facility is within ratio and capacity. Per clerical assistant, there are no firearms present or stored on the premises. The caterpillar room, butterfly room, and dining room were inspected including furnishings, equipment and bathrooms. There is age appropriate furniture, play items and equipment which are free of broken pieces/hazardous conditions. Surfaces including counter tops, floors and shelves are kept free of toxins. There were no hazardous items/toxins observed to be accessible to children in care. Toilets are age appropriate and in working conditions. There are working faucets and water fountains available in both classrooms. Sleeping cots and linens are stored individually and identified by child. There are individual storage cubbies for children's personal belongings. Children were directly supervised by staff. The facility has a centralized fire alarm system which includes carbon monoxide detector. There are multiple fully charged fire extinguishers present. First aid supplies were inspected. Per clerical assistant, the most recent fire department inspection was within the past year. This facility serves breakfast, lunch and snack to children in care. The meals/snacks are prepared on site by kitchen staff. Food prep and storage areas were inspected for cleanliness and absence of pests. There are menus posted for public review. Required postings are present. The outside play are is fully fenced and available for children in care. The separately fenced, farthest back play area is off limits to children in care and per clerical assistant is not used by children in care. There are no pools or other accessible bodies of water present. The slides/swings/high climbing equipment are securely anchored and have cushioning under/around. LPA reviewed the facility, personnel and children's records including staff background clearances and qualifications, children's emergency ID forms, parent's rights forms and immunization records. Two teacher qualification forms were completed during this inspection.
