25 | On 11/4/22 at 9:45am, Licensing Program Manager (LPM) Loretta Dyson arrived at the facility for an unannounced required - 1 year inspection. LPM met with Head Teacher Karen Choi. There were 12 children and one additional staff member also present. The family advocate was also present. This program operates in two sessions, 8:30 - 11:30am and 12 - 3pm, Monday - Friday in the portable near the corner of Bancroft Avenue and 98th Avenue. The portable is on the campus of Reach Academy. During the PM session there were 11 children and 2 staff present.
LPM completed a tour of the portable and the outdoor space used for the center, to conduct a health and safety inspection. LPM observed that the portable has sufficient heating, lighting and ventilation for the safety and comfort of children and staff. The floors and surfaces appear to be safe and in good repair. There is an ample supply of age appropriate furniture, toys, activities and equipment which appear to be in good condition and safe, inside of the portable. There is adequate storage for children's belongings. The bathroom inside of the classroom has 2 toilets and 2 sinks which are in sanitary and operational condition. There is one additional sink inside of the classroom area. There is a separate bathroom for staff inside of the portable. Breakfast is provided for children attending the AM session, and lunch is provided for children attending the PM session. The meals are provided by the school cafeteria. There is a refrigerator inside of the office area and LPM observed it to be clean, with no expired or spoiled food observed. The facility serves 1% milk to children. The facility has a menu, which is dated at least a week in advance, posted on the board. Children bring individual water bottles from home and they are refilled as needed. There is a drinking faucet inside of the classroom. LPM observed that all containers used for storage of solid waste are in good repair and have a tight fitting cover that is kept on. LPM did not observe any bodies of water, hazardous items, toxins or medication accessible to children today. The outdoor play space is right outside of the portable, and is fully fenced. There is sufficient cushioning underneath the play structure to absorb a fall, and LPM observed that the play structure is secure. All required forms are posted. The disaster drills are conducted with the elementary school and the last drill was conducted on 10/31/22. The center is equipped with first aid supplies, a working telephone, centralized smoke detection system, carbon monoxide detector and a fully charged 3A40BC fire extinguisher. |
