On November 17, 2021 at approximately 8:10am, LPA Haderer arrived unannounced for an annual inspection for compliance to Health and Safety. The new CARE tool was used for the inspection. Present for the inspection was the site director Ida Gemignani, 5 teachers and 15 infant children in care.

At 8:30am LPA initiated the inspection of classroom 1 and 2 and at 9:00am stopped to conduct a Case Management investigation for another issue. At 11:45am, LPA resumed the annual inspection of the infant center.

The infant center is located in Rooms 1 (children up to 12 months) and Room 2 (12 to 24 months). All 0-12 month infants in care had the Individual Infant Safe Sleeping Plan form LIC9224 signed by each parent and present in the classroom. Infant Sleep logs were available to track and monitor sleeping infants.



The children have their own school yard and playground, all is in safe condition and free from sharp, loose or pointed parts. There are bubble mirrors installed to assist teachers to see the full playground area. Access to drinking water is available and teachers are always present.

The facility has fully charged 2A10BC fire extinguishers, in classroom. The last annual inspection was done 1/22/2021. Fremont Fire Department conducts fire inspections, the last inspection was completed12/19/2019, independent alarm inspections were done by Century Fire Protection 2/1/2021 and SSD Alarm 7/29/2021. Carbon monoxide and smoke detectors were available for the adjoining classrooms 1 and 2. Heating and ventilation in the classrooms are acceptable.

Changing tables are available in each classroom (within arm’s length of the sink). There are enough towels and soap supplies. The staff have separate adult bathroom located in the school office.

The facility is clean and well organized with ample age appropriate furnishings and equipment. Surfaces including floors and counter tops are clean and toxic free. The school follows proper Covid-19 protocols, parents and visitors are not allowed, children are wearing face coverings. Hazardous items/toxins are kept in staff storerooms out of the access of children. There are no bodies of water accessible to children in care. The facility provides snacks and lunch (including formula and milk) for the children, menus were appropriately posted.

Disaster drills are conducted at least once every six months, the last drill was conducted 9/30/3021 (fire) and 10/17/2021 (earthquake).

The sign in/out sheets were reviewed and are accurate. Classrooms have trash cans with tight fitting cover for the disposal of solid waste. There are no infant children that require incidental medical service.

Children's records were reviewed: LPA requested and reviewed facility roster. All files selected were complete and up to date.

Staff files were reviewed. Opening and closing staff have current CPR and first aid training. All staff subjected to criminal review have been cleared and associated to the facility. Some staff files were missing proof of flu shot, it is time for flu shots now and being scheduled.

All documents required to be posted were appropriately posted on the walls: License; Emergency Disaster Plan; Earthquake Preparedness checklist; Notification of Parents Rights; Personal rights; Child seatbelt laws; menus; daily activity schedules.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Site Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
