An unannounced Annual Site inspection was conducted on this date by Licensing Program Analyst (LPA) Brittany Newton and Licensing Program Manager (LPM) Sherelle Johnson. Upon arrival, at 9:45 AM LPA met Assistant Director Shawinder Brar who helped LPA and LPM complete the tour. Administrator Haneefah Shuaibe arrived at 10:15 AM. There were 26 Infants present being supervised by 9 staff. A complete tour of the facility was conducted. This program currently operates from: Monday - Friday 7:30 AM - 6:00 PM.

Two Infant rooms were inspected (Caterpillars and Busy Bees) for health and safety inspection. The facility appears to be safe, sanitary and in good repair. All cleaning products are stored and made inaccessible to children. All furniture appears to be in good condition. Infant changing tables are placed within arm’s reach of a sink and the sinks dispose hot water for sanitation. There is adequate storage for children's belongings. The toys appear to be safe and well maintained. The food preparation area and storage areas are clean and in sanitary condition. A menu is posted and is visible for review. Infant bottles are appropriately labeled. Drinking water is available for both indoors and outdoors. The facility has a carbon monoxide detector, fire extinguisher and first aid kit stored in the classrooms. The facility has appropriate napping equipment for napping children. Outdoor activity space provided for infants is in good repair. The isolation area is the staff lounge.

Outdoor equipment was inspected for safety, cushioning material, good repair and age appropriateness. Required shade, drinking water and fencing were inspected. Play area was inspected for hazards and inaccessibility to bodies of water.

Snack/lunch menu is posted and available for review. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. There are tight fitting lids on trash cans in the classrooms. First Aid supplies are being maintained. Sign In/Sign Out pads were reviewed.

Teacher - child ratios were observed, and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met, which LPA verified it was. Staff certification in CPR and FIRST AID is current and valid for at least one staff member at this site. Staff records were reviewed at 10:15 AM for completeness including but not limited to: Criminal Record Clearances for adults and Teacher Qualifications. Children’s records were reviewed during today's visit to verify separate, complete, and current records are being maintained for each child in care.


The last fire drill was conducted October 2, 2019.

Incidental Medical Services (IMS) policy was discussed. Licensee reminded when any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Licensee was reminded that anyone working, residing or frequently visiting the facility must be fingerprint cleared prior to being in the presence of children, or an immediate civil penalty can be assessed.


Notice of Site Visit was issued and posted. This notice is to be posted for 30 consecutive days. Failure to keep this notice posted for the 30 consecutive days will result in an immediate $100 civil penalty.
Facility was cited
101170(e)(2) Criminal Record Clearance and three Technical Assistance for napping equipment, infant needs and services plan, and classroom temperatures. A Civil Penalty for $100 was assessed for the lack of criminal record clearance transfer for one staff. for A copy of this report shall be maintained for 3 years and available for public review upon request. Exit interview conducted and a copy of this report was left with Administrator Haneefah Shuaribe and Assistant Director Shawinder Brar.
