25 | On 2/1/2022, Licensing Program Analyst (LPA) Melanie Otsuji, met with licensee Purba Zaman for an UNANNOUNCED 1 YEAR REQUIRED INSPECTION. Present for this visit were 2 fingerprint cleared adults, and 8 children (4 infants, 4 preschoolers). The home was toured to conduct a Health and Safety Inspection. The facility currently operates Monday through Friday from 8:00am until 6:00pm.
The home is single story. The home consists of a living room, dining room, family room, 2 bathrooms, 3 bedrooms, garage and kitchen. The living room, dining room, family room and bathroom at the end of the hallway are on limits. Off limit areas are barricaded by way of closed doors. Outdoor play area is fenced and contains age appropriate play materials. There are toys and learning materials. There are no pools, hot tubs or any other bodies of water on the premises during today’s inspection. The home has a fully charged fire extinguisher, working smoke detector (nest system), working carbon monoxide detector (nest system), and working telephone. The licensee CPR and First Aid certificate is current and expires 12/12/2022. Per licensee, there are no firearms in the home. 8 children files were reviewed, facility roster reviewed and copy obtained. The licensee is in ratio today. All REQUIRED forms are posted and visible for public review.
Incidental Medical Services (IMS) policy was discussed. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department.
