LPAs reminded the Director of the following: Mandated Reporter training and CPR/First Aid training certification is to be renewed every 2 years.
Acknowledgement of Receipt of Licensing Reports (LIC9224): Pertaining to the Type A deficiency cited on 2/12/2020 during the case management inspection, Director showed proof of signed LIC9224 forms from all parents or authorized representatives of currently enrolled children. Director understands all parents or authorized representatives of currently enrolled children and future enrolling children that may be enrolling and/or enrolled for the next 12 months must receive a copy of report from 2/12/2020 and must sign LIC9224, which acknowledges they received a copy of report.
The Director is reminded that ALL Staff must be fingerprint cleared prior to being in the presence of children in care, or an immediate civil penalty will be assessed from $100 to $3000 per person, per incident.
California Law requires Child Care Center personnel staff to report unusual incidents or injuries to children in care to the child's parents and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624B). Do not leave an Unusual Incident Report on the Licensing Program Analyst voice mail.
There are no deficiencies cited today. An exit interview was conducted with Director Lorena R. Alejandre. Copy of report and appeal rights provided.
This report shall remain on file for 3 years. A Notice of Site visit was given at time of inspection and requested Licensee to post the Notice in an unobstructed, viewable public area for 30 days. |
