On 12/10/21 at 9:15am, Licensing Program Analyst (LPA) Christina Uribe conducted an Unannounced Annual Required Inspection. LPA met with licensee Soni Singh, also present at the time of the inspection were 4 staff & 35 children. The facility is within ratio & capacity compliance today. LPA provided facility representative the Entrance Checklist (LIC 125). The facility was toured to conduct a Health and Safety inspection. The facility currently operates 7a-6p, Monday-Friday in the Busy Bees, Wise Wizards, & Shining Stars Classrooms.

Classrooms: All classrooms were inspected for age-appropriate furnishings, equipment, & adequate storage for children’s belongings. LPA observed the cleanliness of floors & surfaces, the presence of a fully functional carbon monoxide detector, smoke detector, and a fully charged 2A10BC fire extinguisher that is accessible throughout the facility. The center is equipped with a fully stocked first-aid kit and available in the classrooms.

Food Service Areas: All center provided food items are properly labeled & stored separately from cleaning supplies. Food prep area is clean, adequately equipped, & free from hazardous materials. Snack menu is posted one week in advance, available for review, & dated.

Bathrooms: Facility has separate staff and child designated bathrooms. Toilets and faucets are in safe and sanitary operating condition. The children are able to reach the sinks and toilets. Supplies are available to children. Hot water faucets are turned off at this time.

Outdoor Play Area: There are no bodies of water, or free-standing water accessible to children. There are age appropriate toys and materials for the children. The playground outside is fenced. Two play structures were found to be not properly secured to the ground resulting in a Type B Violation.

Based on observation, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. The rope ladder to on the Large Lifetime play structure & all of the geometric shaped climbing play structure are not properly secured to the ground.

Licensee has immediately made these two items off-limits to all children and staff. Licensee will have the items either secured to the ground or removed from the outdoor play area entirely no later than 01/07/2022. Once the items are secured, licensee will notify LPA Uribe and LPA will come out to inspect the correction and clear the deficiency. Children are not to play on the structure until after LPA Uribe has come to inspect the structures.

Based on record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA Uribe observed that out of the 10 children's files reviewed, 9 of them had at least one incomplete licensing form. LPA Uribe discussed in depth which child was missing what information on which forms with the licensee.

Licensee will thoroughly inspect all children's files and carefully read each form to ensure that all are filled out properly and correctly. Licensee will also correct the missing information on the children's forms which were reveiwed during today's inspection no later than 01/07/2022.
Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.

Records: All individuals subject to criminal record review have a clearance and have been associated to the facility. LPA reviewed 10 children’s files and 4 staff files around 10:15am. LPA reviewed the facility roster & personnel record. At least one opening/closing staff member has a current Pediatric CPR/First-Aid Certification. Mandated Reporter certificates were reviewed. The center is in compliance with sign in and out procedure. Emergency Drills are recorded and performed every six months. Per facility representative, there are no firearms on the premises. All required documents are posted in a publicly accessible area.

Health-Related Services: This facility does provide Incidental Medical Services (IMS) but does not have any children utilizing the services at this time. Violations Cited During Today's Visit: (Please see attached deficiencies forms)

  • Type B Violation: Two play structures in the outdoor play area are not properly secured to the ground.
  • Type B Violation: Children's records are not properly maintained. Multiple children's files were missing required information on licensing forms.
  • Technical Violation: One staff member very briefly stepped out of the classroom which she was the only present staff member.
  • Technical Assistance: Children (and some staff) not wearing their masks while indoors during non-eating, non-drinking, non-napping activities.

Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Effective August 1, 2003 California Law requires Child Care Licensees to report unusual incidents or injuries to children in care to child’s parents and to the Department of Social Services using the Unusual Incident/Injury Form (LIC 624). Incidents must be reported within 24 hours to the regional office by phone and the written report, LIC 624, within 7 business days.
