On 6/15/2023 at 12:00pm, Licensing Program Analyst (LPA) Catherine Fernandes met with Licensee Heesook Kim for an Unannounced Required Annual Inspection. Present during the inspection were five infants and seven preschoolers. Residing in the home is Licensee and fingerprint cleared husband. Licensee’s home was toured for a health and safety inspection. The facility operates 6:45am – 5:30pm, Monday - Friday.

The home is a tri-level home that consists of three bedrooms and two bathrooms. The entrance to the day care is the front door. The inside and outside of the home were observed to be neat, clean with age-appropriate materials and toys for the children. All toxins, cleaning products, medications, and hazardous materials were observed to be in inaccessible areas. During today’s inspection, LPA observed the following precautions, accessible cabinets and drawers in the bathroom have safety latches and the stairs have a gates to prevent children from falling. Licensee has stated that there are no firearms and there is a pet dog in the home that is around the children in care.


ON LIMITS AREA: The living room, the kitchen, the dining room, the bathroom on the main floor, the family room on the lower level, the first bedroom on the left side and the fenced in backyard.
OFF LIMITS AREA: the bedrooms, the bathroom in the master and the left and right side of the backyard, which will be inaccessible by closed and/or locked doors or visual supervision.
ISOLATION AREA: on the couch in the living room

The home has a fully charged 3A40BC fire extinguisher located on the wall in the kitchen, push botton fire alarm next to the front door and a working smoke detector in the kitchen. Licensee has a working telephone, and all required forms were printed and posted. The Licensee's CPR and First Aid certificate is current and expires on 11/23. LPA did observe any bodies of water located behind or in the home. LPA reviewed 12 children’s files and two staff files.

During today's inspection LPA observed the following deficiencies:
Upon arrival at 12:00PM, LPA asked how many children were in care and Licensee stated 11 children with four infants. LPA then asked where her assistant was at? Licensee stated "her husband went to the store to get milk".
At 12:22pm, the licensee's husband arrived to the home.
At 12:10pm, LPA asked for the a children's roster and the children's files. Licensee stated "that the roster needs to be updated.
During the file reviews 12 out of 12 children did not have a complete file and were missing immunization's.
Three out of the three infants under 12 months did not have infant sleep plans and infant sleep logs.
Adults in the home need proof immunization's and mandated reporter training.
There were no documented disaster drills

Licensee was reminded that California Law requires Licensee to report unusual incidents or injuries to children in care, to child's parents, and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). Incidents must be reported within 24 hours by phone, fax, or email. LPA informed Licensee that all forms can be downloaded at www.ccld.ca.gov. Licensee was also informed that Mandated Reporter Training ("General" and "Child Care Providers") is required for all staff and is to be renewed every two (2) years by visiting http://www.mandatedreporterca.com. Licensee was reminded that EMSA approved Pediatric CPR & First Aid training must be completed every two (2) years. Children’s Roster must be properly maintained, and fire/disaster drill must be conducted every six (6) months and documented. The Licensee is reminded that any structural changes to the home or additions to the childcare facility must be reported to Community Care Licensing.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA Fernandes informed licensee Heesook Kim that this report dated 6/15/23 documents five Type A citations which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Fernandes informed the licensee to provide a copy of this licensing report dated 6/15/23 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
