25 | On 2/15/2022, Licensing Program Analyst (LPA) Melanie Otsuji, met with licensee Nasreen Rahman for an UNANNOUNCED 1 YEAR REQUIRED INSPECTION. Present were two additional fingerprint cleared staff, Licensee's fingerprint cleared spouse, 4 infants and 6 preschool aged children. The home was toured to conduct a Health and Safety Inspection. The facility currently operates Monday through Friday from 8:00am until 5:30pm.
The home is two story home which consists of 5 bedrooms and 4 bathrooms. The ON LIMIT areas are: downstairs bedroom, downstairs bathroom, sunroom, kitchen/dining room, and living room. All other rooms/areas are off limits including garage and entire second floor. Off limit areas are barricaded by way of closed doors and or gates. There are toys and learning materials. There are no pools, hot tubs or any other bodies of water on the premises during today’s inspection. The home has a fully charged fire extinguisher, working smoke detector, working carbon monoxide detector, and working telephone. Per licensee, there are no firearms in the home. Children files were reviewed, facility roster reviewed and copy obtained. The licensee is in ratio today. All REQUIRED forms are posted and visible for public review.
Incidental Medical Services (IMS) policy was discussed. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department.
