13 | Licensing Program Analyst (LPA) Melanie Otsuji arrived to the facility unannounced to complete investigation into the above allegation. LPA met with Owner/Administrator, Ruth Yong. Also present during today's visit were 5 infant aged children and 3 staff members.
During the course of the investigation LPA conducted interviews and record review. Based on interviews conducted, it was confirmed that facility failed to allow a child to return to campus due to not following the campus' covid protocols which included signing a letter which states "Suppose a classroom must be shut down due to COVID-19 exposure due to you knowingly violating the above policy resulting in your child testing positive for COVID-19. In that case, you could be liable to pay up to two weeks of collective tuition for the classroom." Based on LPAs observations and record review(s), the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. Health and Safety Code 1596.80 is being cited on the attached LIC. 9099D. An exit interview was conducted with Administrator, Ruth Yong. |
