On 11/29/2021 at 8:30AM, Licensing Program Analyst (LPA) Christina Uribe & Licensing Program Manager (LPM) Chandra Charles met with Licensee Tyesha Maryland for an Unannounced Case Management Inspection. Present during today’s visit 0 fingerprint cleared staff members & 0 children are present.

Purpose of Today’s Visit: The purpose of today’s visit is to follow up with a previous inspection to which multiple violations were cited and Plan of Corrections were implemented.

Brief Summary of Previous Visit: On 11/04/2021 LPA Uribe conducted an unannounced annual health and safety inspection at the facility in which the Licensee was not present for. The following day, LPA Uribe emailed Licensee with an overview of the inspection findings and how to proceed with the Plan of Corrections. Licensee was instructed (via email sent to her & as stated on the LIC809 Evaluation Report given to Facility Representative) to make contact with LPA Uribe to clear the deficiencies no later than 11/19/2021. As of 11/19/2021, LPA Uribe had not heard from Licensee at all which prompted this visit today for a Case Management.

At 9:30am during today's inspection, LPA Uribe & LPM Charles began reviewing personnel records & children's files. Notice of Site Visit from 11/04/2021 is posted though needs to be relocated to the outside view for clients and will remain until 12/05/2021. All LIC9224 Acknowledgement of Receipt of Licensing Reports are signed and filed by enrolled children's parents. Refer to LIC809D subforms Cleared POC Letters for deficiencies cleared during today's inspection. Additionally, Licensee has provided receipts for registration for 16 hours of EMSA Health & Safety Training and both will be completed by 12/19/2021. The only deficiency that was not cleared during today's inspection is the failure to ensure the renewal of Mandated Reporter Training for staff members resulting in a civil penalty.

Licensee will complete the Child Care Provider Mandated Reporter Training AB1207 for each staff member.

At 10:30am, LPM Charles reviewed the list of associated individuals with Licensee and LPM removed a list of approved individuals in the Guardian. Licensee is advised to register with Guardian and manage her associated employees. Registration & Additional Information for Guardian at https://guardian.dss.ca.gov/

Licensee told LPA Uribe & LPM Charles of plans of permanent closure of her second facility at 1446 Havenscourt Blvd, Oakland, CA 94621. She has been advised to email that facility's assigned LPA Diana Campos at diana.campos@dss.ca.gov and inform her of the closure date & that no letter is to be distributed to parents as no children have been enrolled or attended this Oakland facility since March 2019.

Licensee was notified of a Non-Compliance Conference (NCC) in which requests her participation for a consultative meeting at our Regional Office scheduled on 12/15/2021 at 2:00PM. Licensee requested for the NCC to be rescheduled for a later date in which she will be more available to attend. LPA Uribe instructed Licensee to send her an email with this request along with dates for which she is available in order to reschedule the NCC. Original letter of NCC was given to Licensee.

A notice of site visit was given and must remain posted for 30 days. Appeal Rights was discussed and given to Licensee. Exit interview conducted and report was reviewed with the licensee, Tyesha Maryland.
