On 01/28/2020, Licensing Program Analyst (LPA) Brittany Newton made an unannounced visit for the purpose of conducting a required annual inspection. LPA was met by Director Heather Stephens-Smalley. Present for this visit was 28 preschool aged children with three teachers including the director.
The center was toured for a health and safety inspection.

CLASSROOM: The entire center was inspected. The center operates in three rooms. There are adequate play and learning materials available. Furniture and equipment is age appropriate and in good repair. There is adequate heating, ventilation and lighting. There are water bottles for each child which is brought by the parents. The center has a drinking fountain available inside and a pitcher of water for outside time There is a working telephone on site. There is proper individual storage space for each child. Napping equipment was observed to be free from cross contamination as there is a separate storage bin for each child. Sign in/out was found accurate for children present. BATHROOMS AND TOILETING AREAS: There are separate bathrooms for staff and children. The staff bathrooms are located in the church. The isolation area for sick children is the director's office. The isolation bathroom is the staff bathroom. Toilets and facets work properly. The children are able to reach the sink and toilets. Toilet paper, soap, and paper towels are all available to the children. Adequate lighting is provided in both bathrooms. Bathrooms have cold water available to the children. INSPECTION OF FOOD SERVICE AREA: The parents provide their children with lunch and snacks each day, but the facility has additional snacks available on hand for children. INSPECTION OF OUTDOOR PLAY AREA: There are age appropriate toys and materials for the children. They is a large play structure and many other play equipment available for use. The center has grapevines and gardens that the children tend too with the staff. HEALTH RELATED SERVICES: Site Director states that there is an epinephrine pen for a child, and a asthma inhaler for another child stored at the center in a locked box. Earthquake emergency items are available in the director's office. A first aid kit is also available. Pediatric CPR and First Aid certificates are current and expire on February 28, 2020. Facility has been conducting fire drills with the last one being 12/03/2019.

The facility has a fully charged 2A10BC fire extinguisher. RECORDS: Required forms were posted in an public accessible area. File review of staff and children showed compliance.

Incidental Medical Services (IMS) policy was discussed. The facility is currently providing Incidental Medical Services for administering epinephrine injections and inhalers. LPA Newton reviewed storage of medication and consulted with Director. LPA additionally discussed the need to submit a plan of operation for providing IMS services. Facility is reminded that ALL assistants, volunteers, frequent visitors, or adults living in the home, that are 18 years of age or older must be fingerprint cleared and associated to this facility prior to being in the presence of children in care or an immediate civil penalty will be assessed from $100 to $3000 per person, per incident. Licensee was reminded of the responsibility as a mandated reporter and the requirement to complete the training every two years.



A copy of the child care roster was obtained. LPA Newton provided director with a personnel report which was completed and retained.

No deficiencies observed at this visit. A Notice of Site visit was given and facility was reminded that it is required to be posted for 30 days. Exit interview conducted, appeal rights provided, and a copy of this report was left with director Heather Stephens-Smalley.
