25 | On 7/26/2022 Licensing Program Analyst (LPA) Jaylena Miller, met with licensee Lisa Hendricks for an UNANNOUNCED ANNUAL REQUIRED INSPECTION. Present for the inspection were licensees fingerprint cleared and associated wife (assistant) Sally Scrutchin, 3 infants, 4 preschoolers and 3 school age, and the licensee is within ratio today. Upon arrival LPA provided licensee a copy of form LIC 126. The home was toured to conduct a Health and Safety Inspection. The facility currently operates Monday-Friday 7:00am-6:00pm.
The home is a single story home with 3 bedrooms, 2 bathrooms, living room, kitchen, dining room, garage and back yard. LPA observed the home to be neat and clean with central heating and ventilation for safety and comfort.
The OFF-LIMIT AREAS are the master bathroom and the garage, and will be inaccessible to children by locked doors, safety gates and visual supervision.
The ON-LIMIT AREAS are the living room and dining room which are used as the main day care areas, the kitchen, hallway bathroom, and all three bedrooms that will be used for nap time only. The designated isolation area will be an area in the living room. The backyard will be a designated outdoor play area that is fully fenced. The outdoor area has age appropriate toys and furnishings that LPA observed to be clean and free from defects and dangerous conditions. There is a swing set the LPA checked to be stable and secured for children and LPA reminded licensee to check it frequently for stability and that it is secured. There is also monkey bars that is not used for day care children and it fenced off to prevent access to daycare children.
All hazardous materials and toxins are kept out of reach from children and are not accessible. The home has a fully charged fire extinguisher 2A-10-BC, working smoke/carbon monoxide detector combo, telephone and fully stocked first aid kit. There are no pools, hot tubes or any other bodies of water present at the time of the inspection. Per licensee, there are no firearms on the premises.
