Licensing Program Analysts Arminder Singh and Caroline Colson met with Lisa Luoh center director, for an unannounced random annual inspection at 02:00 PM. There are eight classroom, a computer lab and a playroom for the entire program. C1 and C2 had complete children's files. S1 and S2 had complete immunization records. There are no additional records for review. Present in the facility were 64 school age children with 8 staff members which includes two teachers and one director. Sign-in and sign-out sheets were reviewed to verify census and signatures. The facility is in good repair. CLASSROOMS: The entire center was inspected. There are adequate play and learning materials available. Furniture and equipment is age appropriate and in good repair. There is adequate heating, ventilation and lighting. Drinking water is available inside and outside the center. There is a working telephone on site. There is proper individual storage space for each child. There are separate bathrooms for staff. The isolation area for sick children is located in the director's office. The isolation bathroom is the staff bathroom which is located near the last classroom. BATHROOMS AND TOILETING AREAS: Toilets and faucets work properly. The children are able to reach the sink. Toilet paper, soap, and a hand dryer are all available to the children. Adequate lighting is provided in both bathrooms. The facility has cold and warm water available to the children. INSPECTION OF FOOD SERVICE AREA: The facility provides only serves one snack. Food was inspected for freshness and quantity. INSPECTION OF OUTDOOR PLAY AREA: There are age appropriate toys and materials for the children. HEALTH RELATED SERVICES: Center Director states that there are no medications stored at the center. Earthquake/ disaster drills are conducted every month. The first aid kit is complete and available.

RECORDS: Staff members and children's files were reviewed. Required forms were posted in an public accessible area. CPR and First Aid certificates are current and expire on 03/18/2020. There are no bodies of water accessible to children. There are no firearms on the premises. All storage areas for poisons are locked. Pest Integrated Management and Mandated Reporting was discussed. The website is www.mandatedreporterca.com. Mandated Reporter Training certificated will be submitted in 30 days.

This facility is not providing Incidental Medical Services-IMS at this time. LPAs discussed IMS services and the requirement to update the plan of operation. Specifics on the plan can be found in the child care center evaluator manual (CCC EM) Policy 101173.

The childcareadvocatesprogram@dss.ca.gov is the email address for the applicant to sign up to receive PINS.

Licensee was asked to go to our website at www.ccld.ca.gov to ensure that she is informed of all new laws and regulations.

The attached type A deficiencies are being cited today and must be corrected by the due date. In addition, an attached type B deficiency is being cited during today's inspection. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. All parents/guardians must sign an acknowledgement form of proof of receiving this report (LIC 9224). The LIC 9224 must be placed in the child's file to be reviewed by licensing.

A site notice was posted. An exit interview was conducted. Appeal rights were discussed and given. This report must remain available for public review for 3 years.

Responsibility for Providing Care and Supervision
