On May 12, 2022 at approximately 9:30am, LPA Haderer arrived unannounced for an annual inspection for compliance to Health and Safety. The new CARE tool was used for the inspection. Present for the inspection was center director Mavi Gomez, 14 staff (Center Director, 12 teachers, 1 admin) with 71 children in care (2 years up to 6 years old). There is no toddler option, the center is in ratio today. The hours of operation are 7:00am to 6:00pm.

There are 6 classrooms: room #1 for age 2 to 2 ½; room #2 for age 2-3; room #3 for age 3-4; room #4 for age 4-5; room #5 and #6 for age 4 ½ to 6. LPA observed that all preschool rooms are clean and safe for all children and staff. Drinking water is readily available for the children in each room (they also bring their own water bottles from home). Water is brought outside to the playground area.



The facility is clean and well organized with ample age appropriate furnishings and equipment. Surfaces including floors and counter tops are clean and toxic free. There were no hazardous items/toxins observed to be accessible to children in care today. There are no bodies of water accessible to children in care.

LPA observed solid waste containers with tight-fitting lids. The facility has four fully charged 2A10BC fire extinguishers, the last annual inspection was completed on 7/23/2021. Carbon monoxide detectors were present, tested and working. Arrow Fire Protection completed testing of fire alarm system. The last test was completed on 7/26/2021 all tests passed inspection. Heating and ventilation is acceptable. Disaster drills are conducted every month, the last drill was conducted 4/22/2022. There are alarmed fire exit doors. Each classroom has cubbies for children’s personal items.

The sign in/out sheets were reviewed and are accurate. All children present were signed in.

There is a staff only kitchen area consisting of a small refrigerator, a sink with hot and cold running water, microwave, a stove and a no-touch trash bin with a cover. Currently the school only provides snacks, no lunches. Parents supply their own child’s lunch. There are some fruits available and snack foods in a tall cupboard. All snack foods in the cupboard are stored in containers with tight fitting lids or in plastic re-sealable bags. There were no sharp knives observed in the kitchen today.

Any medications at the facility are stored in the emergency first aid backpacks out of the reach of children. Medications were checked and all have required signed parental forms. There were no expired medications found.

There is one children’s bathroom and 2 staff bathrooms. The children’s bathroom has 4 sinks with functioning cold water taps with appropriate hand washing signs. There are 8 toilets for the children (4 groups of 2 each with a wall barrier to separate them from each other). There are enough towels and soap supplies. All toilets, handwashing and cleaning areas are in safe and sanitary operating condition. There is a touch-free trash can with a lid and sufficient amount of towels, soap and diaper supplies. There is a changing table within arms length of the sink.

LPA observed sufficient pads for the children to sleep. Center director stated that bed sheets were taken home and washed by parents once every week. Center Director stated that there are no weapons or firearms on the premises.



There is a large playground area in the back of the center surrounded by a 6-foot privacy fence. There are two locked exit gates, secured with inside latches too high for children to reach. The playground is in safe condition and free from sharp, loose or pointed parts and the areas around or under high climbing equipment has appropriate rubber cushioned material that absorbs a fall and enclosed within the school property. Ample shade is available for the children in the play areas and teachers are always present. Water is brought outside for the children.

Children's records were reviewed: LPA requested and reviewed facility roster. All files selected were complete and up to date. At 12:10pm Staff files were reviewed. Opening and closing staff have current CPR and first aid training. All staff subjected to criminal review have been cleared and associated to the facility. One teacher’s Mandated Reporter certificate recently expired, Technical Violation issued. All files were complete, up to date and found to be in compliance with Title 22 regulations. Immunization records were available showing all teaches files were complete.

All documents required to be posted were appropriately posted on the walls: License; Emergency Disaster Plan; Earthquake Preparedness checklist; Notification of Parents Rights; Personal rights; Child seatbelt laws; menus; daily activity schedules. There is a posted waiver for this facility for the playground space to be alternated to limit the number of children.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

There were no deficiencies issued today. This report will remain on file for 3 years.
