On 12/06/2021 at 9:00am, Licensing Program Analyst (LPA) Christina Uribe & Licensing Program Manager (LPM) Chandra Charles conducted an Unannounced Annual Required Inspection. LPA met with Licnesee Urvashi Jagga, also present at the time of the inspection were 9 staff & 59 children. The facility is within ratio & capacity compliance today. LPA provided facility representative the Entrance Checklist (LIC 125). The facility was toured to conduct a Health and Safety inspection. The facility currently operates 8:30am-5:30pm, Monday-Friday in 4 classrooms (Ant Room, Busy Bee Room, Butterfly Room, & Ladybug Room).

Classrooms: All classrooms were inspected for age-appropriate furnishings, equipment, & adequate storage for children’s belongings. LPA observed the cleanliness of floors & surfaces, the presence of a fully functional carbon monoxide detector, smoke detector/fire alarms, and a fully charged 3A40BC fire extinguisher that is accessible throughout the facility. The center is equipped with a fully stocked first-aid kit and available in the classrooms.

Food Service Areas: All center provided food items are properly labeled & stored separately from cleaning supplies. Food prep area is clean, adequately equipped, & free from hazardous materials. Snack menu is posted one week in advance, available for review, & dated. LPM did find that facility is serving 2% milk to children when only 1% or nonfat is allowed.

Bathrooms: Facility has separate staff and child designated bathrooms. Toilets and faucets are in safe and sanitary operating condition. The children are able to reach the sinks and toilets. Supplies are available to children.

Outdoor Play Area: There are no bodies of water, or free-standing water accessible to children. There are age appropriate toys and materials for the children. The playground outside is fenced and all equipment and surfaces are free from hazards.

Records: 3 individuals subject to criminal record review do not have a clearance and have not been associated to the facility. LPA reviewed 8 children’s files & 5 staff files around 12pm where violations were assessed. Facility roster & Personnel Record were not prepared or available for review during today's inspection. At least one opening/closing staff member has a current Pediatric CPR/First-Aid Certification. Mandated Reporter certificates were reviewed. Director’s CPR/First-Aid Certificate is current and expires on 07/21/2023. The center is in compliance with sign in and out procedure. Emergency Drills are recorded and performed every six months. Per facility representative, there are no firearms on the premises. All required documents are posted in a publicly accessible area.

Violations Cited Today:

  • Type A Violation: S1, S2, & S3 did not have proper association to this facility.
  • Type B Violation: S1, S4, & S5 did not have an up to date Mandated Reporter Training on file
  • Technical Violation: C1, C2, & C3 had incomplete Consent for Emergency Medical Treatment forms (LIC627)
  • Technical Violation: C2 had an incomplete Information & Emergency form (LIC 700) as the child's Physician to be contacted in an emergency was not filled out.
  • Technical Violation: Personnel Report & Facility Roster was not maintained or available for review.
  • Technical Violation: S1 & S2 do not have a Health Screening Report (LIC503) included in personnel files.
  • Technical Violation: S1, S2, & S4 do not have proof of immunizations in their personnel files.
  • Technical Violation: C4 did not have a Physician's Report (LIC 701) included in their file for review.

Health-Related Services: This facility does provide Incidental Medical Services (IMS). LPA inspected storage of medications and equipment/supplies, and reviewed children’s, personnel, and administrative records. Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee [or facility representative] and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Effective August 1, 2003 California Law requires Child Care Licensees to report unusual incidents or injuries to children in care to child’s parents and to the Department of Social Services using the Unusual Incident/Injury Form (LIC 624). Incidents must be reported within 24 hours to the regional office by phone and the written report, LIC 624, within 7 business days.

The attached type A violations is cited today and must be corrected by the due date. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians newly enrolled at the facility during the next 12 months. All parents/guardians must sign an acknowledgment form of proof of receiving this report (LIC9224). The LIC 9224 must be placed in the child's file to be reviewed by licensing.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee, Urvashi Jagga.
