On November 2, 2022 at approximately 10:50 am, LPA Haderer arrived unannounced for an annual inspection for compliance to Health and Safety. The new CARE tool was used for the inspection. Present for the inspection was the site director Madhu Gupta, her husband Rajesh Gupta 4 teachers, and 28 children in care. The facility is in ratio today.

The center is operated out of a converted home, it has 4 classrooms, an office, an entry area, kitchen, one large restroom for the children with 4 toilets and sinks, another small children’s restroom and a separate staff bathroom. A wall that separated one classroom (room 3) and a nap room (room 4) has been removed. This has slightly enlarged the two separate rooms into a bigger single room.



There is a large outside enclosed play area for the children surrounded by a 6 foot wooden fence. All playground structures are in safe condition and free from sharp, loose or pointed parts. Areas around and under high climbing equipment, swings, slides and other similar equipment are cushioned with material that absorbs falls. Access to ample shade and drinking water is available and teachers are always present.

The facility is clean and well organized with ample age appropriate furnishings and equipment. Surfaces including floors and counter tops are clean and toxic free. Hazardous items/toxins are kept in staff storerooms out of the access of children. There were no bodies of water accessible to children in care. The school provides snacks (including formula and milk) and optional hot lunches for the children, menus were appropriately posted. All dry good foods were checked, none were expired. Milk served is non-fat, which complies with healthful food and drink requirements.

The sign in/out sheets were reviewed and are accurate. Classrooms have trash cans with tight fitting covers for the disposal of solid waste. There are no children that require incidental medical service.

The facility has fully charged 3A40BC fire extinguishers in the classrooms and kitchen area. The last annual inspection was done 2/25/2022. ADT Commercial (formerly Protection One) conducts fire system inspections, the last inspection was completed 10/23/2021, the system passed. Carbon monoxide and smoke detectors were available, working and tested. Heating and ventilation in the classrooms are acceptable. Disaster drills are conducted at least once every six months, the last drill was conducted 8/22/2022.

Children's records were reviewed: LPA requested and reviewed a facility roster. All files selected were complete and in good order.

Staff files were reviewed. At least one staff member who is trained in pediatric cardiopulmonary resuscitation and pediatric first aid training. All staff subjected to criminal review have been cleared and associated to the facility. Immunization records for all staff were present and in compliance with child care center requirements.

All documents required to be posted were appropriately posted on the walls: License; Emergency Disaster Plan; Earthquake Preparedness checklist; Notification of Parents Rights; Personal rights; Child seatbelt laws; menus; daily activity schedules.

Site Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
