32 | The facility is operating within their licensed capacity and is in ratio today. All required postings are made visible on the window by the back entrance. All teachers present have obtained a criminal record clearance, exemption or transfer. The fire/disaster drill log is complete with the last drill logged on 5/23/2023. A physical census of the children was taken and cross referenced with the sign-in and out logs. All children have been properly signed in by their parent or authorized representative. LPA obtained a sample of the children’s files, the staff files, and the facility files. Through record review it was found that out of the eight (8) staff files reviewed four (4) teachers were missing LIC503 Health Screening Report, four (4) were missing a record of a TB test, three (3) were missing immunization records for MMr, Tdap and Flu, and two (2) staff members had a missing or expired Mandated Reporter certificate. LPA also found that the on site cook does not have a criminal record clearance.
Deficiencies cited during inspection
· Adult present without a criminal record clearance
· Four (4) staff missing Health Screening Reports and TB tests on file
· Three (3) staff missing immunization record on file
· Two (2) staff missing a valid Mandated Reporter certificate on file
Director was reminded that EMSA approved Pediatric CPR & First Aid training must be completed every two (2) years. Mandated Reporter Training ("Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting https://mandatedreporterca.com/. Personnel and facility roster must be properly maintained, and fire/disaster drills must be conducted at least every six (6) months and documented. Licensee was reminded that California law requires facilities to report unusual incidents and/or injuries to children in care, to the child's parents, and to the Department within 24 hours by phone, fax, or email. Within seven (7) days from the incident, facilities must submit the Unusual Incident/Injury form (LIC 624) to the Department. LPA informed Director that all forms can be downloaded at www.ccld.ca.gov. |
