1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	A Case Management Inspection was conducted on this date 9/19/2023 at 1:35PM by Licensing Program Analyst (LPA) Melanie Otsuji. LPA met with Director, Carmina Harris. An application was submitted for an Addition of Rooms and Capacity Increase. The center has requested to add TWO toddler rooms and two garden areas as licensed space and increase capacity from 60 preschool aged children to 92 preschool aged children which includes the option to have 24 of those children be enrolled into the Toddler Option program (18 months-36 months of age). The program is currently utilizing 1 large classroom split into 2 classrooms on site of the St. James Catholic Church. Operating hours are Monday through Friday from 8AM - 6PM. Present today was the Director, 5 additional staff members and 28 napping preschool aged child. A health and safety inspection was conducted inside and outside. Measurements were taken and are as follows: INDOORS: 3908.07 square feet = 111 children OUTDOORS: 4577.48 square feet = 61 children There are a total of 6 sinks, 6 toilets and 2 urinals available for children use. Facility utilizes the back play yard and the two garden/science areas in front of the school. Drinking water is available inside and outside by way of bottled water and bottles brought from home. Facility provides lunch and AM/PM snacks. Menus are posted. Facility has a functioning carbon monoxide detector, smoke detector and fire extinguisher accessible throughout the facility. Facility utilizes an electronic sign in/out application which provides a unique identifier for each authorized representative. A paper copy is available should the application not work. An approved Fire Clearance was received on 9/6/2023 from Fremont Fire Department for 90 preschool aged children.

All licensing required documents are posted. Per Director, all Opening and Closing staff have current Pediatric CPR/First Aid. This facility plans to provide Incidental Medical Services – IMS. An updated Plan of Operation that includes IMS must be submitted to the Department when any changes are made. The center was found to be clean, safe, sanitary and in good repair. There are no deficiencies cited during today's visit. A license with approval for 90 preschool aged children with the option to have 24 of those be enrolled into the toddler option program (ages 18 months- 36 months of age) will be issued effective today, 9/19/2023.
