Licensing Program Analyst Caroline Colson met with Head Teacher, Peiyi Tseng at 10:08 AM. Director Xin Dong arrived during the inspection. The facility is a converted home with two upstairs bedrooms, living room, kitchen, one upstairs bathroom for staff, one downstairs bathroom for the children, cubby area, deck, fenced back yard and garage. Preschool Program will continue to operate in 3 rooms which includes 1 upstairs bedroom to the left, downstairs living room and the kitchen area. Toddler-Option (18 months to 36 months) operates in the upstairs bedroom to the right. Hours of Operation are from 8:00 AM to 6:00 PM, Monday through Friday. There are 18 preschool children, 5 toddlers and 5 staff members including the director. Two children's records and four staff records were reviewed by the LPA and the licensee on 02/20/20 at 11:31 AM. C1 and C2 have complete records. The Licensee stated to the analyst that there are no additional records available in the facility. A physical census was taken of all children present and crossed referenced with the sign in and out sheets. CLASSROOMS: The entire center was inspected. There are adequate play and learning materials available. Furniture and equipment is age appropriate and in good repair. There is adequate heating, ventilation and lighting. There are water bottles for each child. There is a working telephone on site. There is proper individual storage space for each child. There are separate bathrooms for staff and children. The staff bathroom is located upstairs. The isolation area for sick children is the upstairs bedroom on the right side. The isolation bathroom is the staff bathroom. BATHROOMS AND TOILETING AREAS: Toilets and faucets work properly. The children are able to reach the sink and toilets. Toilet paper, soap, and paper towels are all available to the children. Adequate lighting is provided in both bathrooms. Bathrooms have cold and warm water available to the children. INSPECTION OF FOOD SERVICE AREA: Facility provides lunch and two snacks daily.

INSPECTION OF OUTDOOR PLAY AREA: There are age appropriate toys and materials for the children. They is a large play structure and many other play equipment available for use. HEALTH RELATED SERVICES: Director states that there are no medications stored at the center. Earthquake emergency items are available in the storage area. The first aid kit is complete. Pediatric CPR and First Aid certificates are current and expire on November 30, 2021. RECORDS: Required forms were posted in an public accessible area. Mandated Reporter Training certificates and immunization records will be sent to our department within 30 days.

This facility is not providing Individual Medical Services - IMS at this time. LPA discussed IMS services and the requirement to update the plan of operation. Specifics on the plan can be found in the child care center evaluator manual (CCC EM) Policy 101173.

The childcareadvocatesprogram@dss.ca.gov is the email address for the applicant to sign up to receive PINS.

Licensee was asked to go to our website at www.ccld.ca.gov to ensure that she is informed of all new laws and regulations.

A site notice was posted. An exit interview was conducted. Appeal rights were discussed and given. This report must remain available for public review for 3 years.

The attached type A and B deficiencies are being cited today and must be corrected by the due date. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. All parents/guardians must sign an acknowledgement form of proof of receiving this report (LIC 9224). The LIC 9224 must be placed in the child's file to be reviewed by licensing.

There shall be a qualified teacher for every 12 preschool children in attendance. This requirement was not met as evidenced by observation and records review conducted during today's inspection. The licensee failed to ensure that qualified staff are providing the care and supervision to children between 10:08 AM to 10:25 AM. This poses an immediate risk to the health, safety, or personal rights of children in care. During the inspection, LPA observed when reviewing records that at the time of arrival that the two staff members were aides and not fully qualified teachers.

There are wood planks that have fallen down from the fence. This requirement was not met as evidenced by observation and records review conducted during today's inspection. The licensee failed to replaced the broken wooden planks. This poses an potential risk to the health, safety, or personal rights of children in care. LPA observed the broken wood planks when touring the facility between 10:00 AM and 10:25 AM.
