On June 29, 2023 at 4:20pm Licensing Program Analyst (LPA) Indira Loza and Licensing Program Manager (LPM) Mayla Mendoza met with Ebony Bell to conduct an unannounced Case Management inspection to check a newly installed play structure to ensure its safety. There were 26 children and 7 staff.

The play structure is in the infant/toddler yard, It has two stories, and made of wood. The structure was checked for safety. It was determined that the structure was not anchored to the ground, and was not age appropriate for the infant/toddler children, as well as not manufactured by a professional company.

Facility was instructed to keep children off the play structure until LPA re-visits with the facility's new plan.

Report was reviewed with Ebony Bell.
