On 5/18/2021 at 8:45am, Licensing Program Analyst (LPA) Diana Campos arrived at the home for an announced pre licensing, change of location, inspection. LPA met with Ana Lanuza. There was 1 infant child in care and licensee's own minor nephew who lives in the home. Also present during the inspection was licensee's cousin who lives in the home and her own 2 fingerprint cleared adult sons.

LPA toured the home with the licensee. The home is a single story home, which consists of the living room, dining area, kitchen, 4 bedrooms, 2 bathrooms, basement, and detached garage. The home is neat and clean with heating and ventilation for safety and comfort. The on limit areas include the living room, dining area, kitchen, first bedroom to the right of front door. Bathroom to the right of kitchen, and the front yard. The off limit areas include the second bedroom to the right of living room, the bedroom to the left of bathroom in the hallway, the bedroom to the right of the back door, the bathroom to the left of the back door, the basement, backyard, and the detached garage. These areas are made inaccessible by closed and/or locked doors and visual supervision. The isolation area will be the bedroom to the right of front door away from other children in care. The outdoor play area is the fenced front yard, and it is free from defects or dangerous conditions. There is an ample supply of age appropriate toys and activities accessible to children, and LPA observed that they are in good condition. There are 2 fully charged 3A40BC fire extinguisher, working smoke detector, working carbon monoxide detector, telephone and fully stocked first aid kits. The heater vents are on ceiling. LPA did not observe any bodies of water, hazardous items or toxins accessible to children today. Per the licensee, there are no firearms in the home. Licensee has current CPR and first aid certificates which expire on 01/25/2022. Licensee has proof of completion of the required mandated reporter training, dated 01/06/2020. LPA reviewed the mortgage statement, and it shows control of property.
