08/05/2021, at approximately 8:30 am., Licensing Program Analyst (LPA) Lorraine Dacanay-Breaux and Licensing Program Manager (LPM) Chandra Charles, met with G.Tulsi, Site Supervisor to conduct an Unannounced Annual Inspection of the Preschool /Toddler day-care facility. This facility operating hours are Monday - Friday from 8:30am to 6:00pm. Present during this inspection were 10 (ten) staff and 39 children present during the inspection.

8:45am LPA and LPM and Site Supervisor conducted a Health and Safety tour of the facility inside and outside.

PHYSICAL PLANT

The facility is equipped with multiple working carbon monoxide detectors, multiple wired smoke detectors, five fully charged fire extinguishers and pull-down fire alarm devices. The facility has a working telephone. The parent board is posted in the main lobby and all required postings are present.

LPA & LPM inspected (5) preschool classrooms #3, 4, 5, 6 and 7 (Toddler Option) classrooms. All the classrooms are equipped with age appropriate tables, chairs, toys and preschool materials. Furniture and equipment are being maintained in good conditions. The floors in all rooms were safe and clean. Each classroom has individual bathrooms.

BATHROOMS AND TOILETING AREAS

There are separate bathrooms for staff and children. Toilets and faucets are in safe and sanitary operating condition. The children can reach the sinks and toilets. Bathroom supplies are available to the children.

FOOD SERVICE AREA

The facility kitchen is not accessible to the day-care children, used as a staff break/lunch room. The school does not provide meals and/or snacks. Food is available if a child forgets to bring a their lunch. The food preparation and storage area are being maintain, kept clean and free of litter. Disinfectants, cleaning solutions, poisons and other items that could pose a danger are inaccessible to children to children in care.

OUTSIDE PHYSICAL PLANT

The playground area is fenced, and all equipment and surfaces are safe and free from hazards. All play structures are anchored to the ground, no visual defects or concerns, there is resilient cushioning to absorb falls and partial grassy area. There are no bodies of water, or free-standing water accessible to children. There are age appropriate toys and materials for the children. There are shaded rest areas covered with canopy type shading. Drinking water faucets is accessible to children both indoors/outdoors.

RECORDS REVIEW

9:50 AM LPA & LPM requested to see and review 6 (six) children files and 12 (twelve) employee facility files.

All individuals subject to criminal record review have a clearance or exemption and have been associated to the facility. All Staff present for today's inspection files reviewed have required health screening, employee rights, and criminal records statements. LPM reviewed the facility roster and obtained a copy. At least six staff members have a current CPR & First Aid Certificate. Mandated Reporter Training was discussed. Director's CPR and First Aid certificate is current and expires on 02/27/2023. The center is following the sign in and out procedure. The center uses ProCare electronic application for the sign in/out process. In case of an emergency, the temperature log is used as a back up.



Disaster drills are being conducted at least once every 6 months and the log indicates the last one conducted was on 07/16/2021. Per Site Supervisor, there are no firearms stored on the premises.

HEALTH RELATED SERVICES: IMS is being provided. LPA and LPM inspected the medication, which is stored in a safe place that is inaccessible to children in care. Each of the medications has an unaltered label with the child’s name and date of issuance. The center is equipped with a fully stocked first aid kit that are available in the classroom.

California Law requires Child Care Centers licensees to report unusual incidents or injuries to children in care to child's parents and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). Incidents must be reported within 24 hours by phone, fax, or electronic mail. Roster of the children must be properly maintained, and fire/disaster drill every six months must be documented.

Director is reminded that ALL assistants, volunteers, and staff, that are 18 years of age or older must be fingerprint cleared and associated to this facility prior to being in the presence of children in care or an immediate civil penalty will be assessed from $100 to $3000 per person, per incident. Director was reminded of the responsibility as a mandated reporter. All forms can be downloaded at www.ccld.ca.gov



Incidental Medical Services (IMS) policy was discussed. This facility provides IMS to children in care. Facility is following IMS plan on file. When any changes to the IMS plan is made, an updated Plan for Providing IMS must be submitted to the Department. No deficiency cited during today’s visit. A Notice of Site visit was posted at time of inspection and must remain posted for 30 days. Exit interview conducted with the Site Supervisor. A copy of the report was provided
