On 4/8/22 at 1:20pm, Licensing Program Analyst (LPA) Emilia Grisak conducted an unannounced complaint inspection and met with Site Supervisor Jen Rich, Program Manager Elizabeth Givens & Board Member Sally Miller. It was alleged that an infant was exposed to bleach solution, specifically that children have had bleach spots on clothing.
The Site Supervisor was interviewed on 3/2/22 at 12:23pm and stated that the facility does use a bleach solution for sanitizing, but it is not used around children. The Site Supervisor stated that one child was observed with a bleach mark in the morning and parents were notified but it could not be determined if the bleach mark occurred at the facility or at home. Interviews were conducted with six staff and four parents on 3/2/22, 3/9/22, and 3/10/22 regarding this allegation. It was stated by three out of six staff that a bleach mark was observed one time on a child’s clothing, but it could not be determined where the bleach mark came from. It was stated in interviews with one parent out of four that a bleach mark has been observed on child’s clothing, but it could not be determined where it came from. Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, and the findings are unsubstantiated. An exit interview was conducted.

It was also alleged that children’s belongings were sent home with other children. The Site Supervisor was interviewed on 3/2/22 at 12:23pm and denied the allegation and stated that there was one incident when child C1’s shoes were accidentally taken home by another family, but they were returned the following day. Interviews were conducted with six staff and four parents on 3/2/22, 3/9/22, and 3/10/22 regarding this allegation. It was stated by two out of six staff that there was one occasion when a child’s belongings were taken home by the wrong family but were returned to the correct owner the following day. It was stated in one out of four parent interviews that a child has come home in the wrong shoes. It was stated during parent interviews that when things have gone missing, they always get it back. Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, and the findings are unsubstantiated. An exit interview was conducted.
