32 | The facility director stated that they had communicated with witness (W2) regarding the incident. Director stated that they had wrote up an unusual incident report and notified W2 through a text communication when C1 was bitten. Director stated that they informed child's responsible party that they were not allowing visitors in the classroom due to following the Covid-19 guidelines.
On 8/02/2022 LPA Mendez conducted an interview with the facility’s assistant director. The assistant director stated that they were notified immediately when C1 was bit and was aware that staff were not able to prevent this incident. She stated that they wrote the ouch report and notified witness (W2) through text message and W1 was notified at pick up.
LPA Mendez interviewed witness (W1) on 8/1/22. LPA Mendez asked W1 if they were notified when C1 was bitten during pick up. W1 stated that C1 was seen by a doctor to treat the bite and prescribed medication to prevent infection.
LPA Mendez interviewed witness (W2) on 8/1/22. LPA Mendez addressed the allegation that staff did not prevent daycare child from being bitten by another daycare child on multiple occasions. W2 stated that (C1) was bitten three times on different occasions, the third time C1 was bit, C1 was seen by the doctor to treat the broken skin and prescribed medication to prevent infection.
Three additional staff were interviewed on 8/02/2022. Two of the three staff interviewed stated proper supervision between staff and children was observed during the biting incidents and could not have prevented the incidents from occurring.
LPA Mendez interviewed witness (W1) on 8/1/22, LPA Mendez asked if they were denied entry to the classroom, W1 stated no and had observed other families picking up. W1 stated they were informed by facility director that they were not allowing families inside the classroom due to Covid-19 guidelines.
During today's visit, LPA toured the facility and observed 5 children and 4 staff present in the classroom.
Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, and the findings are unsubstantiated. An exit interview was conducted. The Notice of Site Visit must be posted for 30 days. |
