25 | An unannounced Case Management visit was conducted at the facility by Licensing Program Analyst (LPA) J. Helton on January 4, 2023 at 8:15 AM in response to an Unusual Incident Report received December 27, 2022. LPA met with Director Tamara Wanink. According to the incident report and interviews conducted, sometime between 8:15 AM and 8:30 AM on December 27th a 2 year old child (C1) left the classroom and was discovered standing outside the front door by the window by a incoming parent (A1). Interviews with staff and C1’s parent (P1) support that C1 went unnoticed by Teacher (T1) until he was returned to the classroom by A1. Between the time of 8:15 AM and 8:30 AM T1 was supervising 6 children.
Based on available information, it has been determined that T1 was not properly supervising the children when the incident occurred on December 27, 2022.
The following violation of the California Code of Regulations, Title 22; Division 12, was observed: see LIC809D
Reports citing Type A violations are to be provided to parents/guardians of children currently enrolled and to parents/guardians of children newly enrolled at the facility during the next 12 months. Parent/guardians must sign Form LIC9224, and it is to be kept in each child’s file.

Copy of report, appeal rights and notice of site visit were provided during the visit. Notice of site visit must be posted for 30 days. Failure to comply with posting requirements shall results in an immediate civil penalty of $100.
