25 | A Case Management visit was conducted by Licensing Program Analyst, Wisehart who met with Heather Miller to discuss reporting requirements and the commingling of age groups across licensees.
Upon LPA arrival to the site at 9:15 am the LPA observed the Director, Heather Miller with the school aged children using the outdoor play area. The Director was alone with 24 children, saying that she had an aide who was using a restroom. The Director was over ratio for a few minutes before the aide enjoyed the group.
On 6/21/22 an incident occurred resulting in C1 fracturing an arm. The child was taken to the hospital on 6/22/22 however, the facility did not submit an incident report until 7/18/22 which is outside the 7-day required time frame.
Interviews with 3 out of 3 witnesses indicate that children from different license age groups are commingled often in the morning. Interviews with 1 out of 1 child, indicated children from different license age groups are commingled at the end of the day when staff take the children to the front building. Interviews with 2 out of 2 staff indicated that commingling happens for a short period of time at the start or end of the day when only a few children are present.
Notice of Site Visit shall be posted for 30 days from today's visit.
The following violation(s) of the California Code of Regulations, Title 22; Division 12, were observed: see LIC 809D. Appeal Rights were provided. Reports citing Type A violations are to be provided to parents/guardians of children currently in enrolled and to parents/guardians of children newly enrolled at the facility during the next 12 months. Parents/guardians must sign Form LIC9224 to be kept in each child's file. |
