25 | On 6/5/23 @ 3:40pm a visit was conducted by Licensing Program Analyst's (LPA's) E. Laird in response to an unusual incident that was self-reported in a timely manner by Kylie Miller, facility director. The incident occurred on 6/1/23 at approximately 11:27am. Community Care Licensing Division (CCLD) was notified within 24 hours and unusual incident report was sent in writing within the 7 days as required. Facility director reported that a child (C1) had been outside on the playground for approximately 30 minutes.
LPA E. Laird conducted an interview with facility director Kylie Miller at approximately 3:40pm who stated on 6/1/23 @ 11:27am three teacher aides were taking some children to the bus. Since the teacher aides were unavailable, one of the teachers had three of her students go with another class so they would stay within ratio. Due to miscommunication, the teacher who was receiving the additional three children, misunderstood and thought she only had two coming to her classroom. Due to this misunderstanding, one child (C1) was not counted and was left on the playground. At approximately 11:57am, another teacher discovered C1 was not inside eating lunch. The teacher immediately went outside and found C1 on the playground.
The facility was toured inside and outside, and classroom roster for “bears” was verified.
The following violation of the California Code of Regulations, Tittle 22: Division 12 was observed: Lack of supervision resulting in a child wandering outside. see LIC 809D. Reports citing Type A violations are to be provided to parents/guardians of children currently in enrolled should sign LIC 9224 form and to parents/guardians of children newly enrolled at the facility during the next 12 months.
Notice of Site Visit shall be posted for 30 days from today’s visit.
