A required annual inspection was made to the facility by Licensing Program Analyst (LPA) Sandy Husband. The facility file was reviewed prior to this visit. A review of the personnel report on 11/25/19 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

This is a Title 5 funded program and is operated by Chico Unified School District (CUSD). The facility’s operating hours are 7:45 AM to 5:00 PM, Monday - Friday year round. The facility was toured inside and outside and the floor and yard plan submitted by the licensee were verified. The items which could pose a danger to children (such as detergents, cleaning compounds and medications) were observed to be inaccessible to children. Poisons are locked in the cabinet above the sinks. The facility was free of flies, insects and rodents. The toys, floors, desks and other equipment and surfaces were clean, toxic free, safe and in good condition. The children nap on cots and bedding is washed weekly off-site and stored in the children's individual cubbies throughout the week. There is uncontaminated drinking water available to children both indoors and oudoors; LPA observed individual labeled water canteens are available for the children during indoor and outdoor play. The children’s bathrooms were in sanitary condition. A current menu was posted on the bulletin board. Breakfast, lunch and snack is provided by the facility. There is a working carbon monoxide detector and charged fire extinguisher in the facility. The playground was under construction and was locked during the inspection for safety precautions. The children are being provided alternative outdoor play areas on the elementary school playground when it is not in use by older children to prevent commingling. There were no bodies of water observed on this facility property. The director stated no weapons are stored on site and none were observed. During today's inspection, staffing ratios were being met and 11 children were being supervised by three fully qualified

teachers during nap time. The facility was operating within the licensed capacity. The emergency disaster drill was conducted within the past six months on 11/18/19. At least one staff member (S1) present during the inspection possessed current CPR and First Aid certifications which expires 6/20/21. Eleven children’s records were reviewed at approximately 2:25 PM; Five children were without medical assessments (C2, C4, C5, C10, and C11) and one child (C10) was missing the personal rights form. Three staff records were reviewed at 3:30 PM and contained health screening forms, immunizations, Employee Rights, Mandated Reporter Training, and Education Units. The sign in/out procedure was reviewed and in compliance. This facility is not providing Incidental Medical Services (IMS) to children. The Department’s IMS policy was discussed with the Director.
