A Prelicensing inspection was conducted by LPAs Sandy Husband and Carrie Wisehart in response to an application submitted on 7/5/19. The facility is located on the Parkview Elementary School campus in an adjacent annexed building named the PACE Building and the requested capacity is 24. The fire clearance has been approved for 24 on 8/7/19. The facility will operate all year round, Monday through Friday from 7:45 AM to 5 PM. There is a designated area for the required postings (Parents Personal Rights, Emergency Disaster Plan, Earthquake Preparedness Checklist, Menu, etc.). Cubbies are available for children to use. There is a working smoke alarm, carbon monoxide detector and charged fire extinguisher in the facility. All Exits are marked. The sign-in and sign-out procedure is in a binder located at the front entrance. The applicant understands that full legible signatures are required. A separate bathroom is available for staff use. There are 2 sinks and 1 toilet available inside the classroom. A waiver shall be on file for the use of elementary restrooms which are gender specific. The Site Supervisor understands that visual supervision shall be provided at all times and children are never to use the restroom without a fingerprint cleared, qualified adult. Medications will be kept in a locked box. Cleaning supplies are out of reach of children. Poisons are locked in a janitorial closet. Preschool children shall be visually supervised at all times. Indoor play area was measured for a capacity of 24. There are 24 cots available for children to nap. Bedding will be laundered by staff off-site, on a weekly basis. Food will be provided to children on site. Breakfast, Lunch, and PM snack will be provided by the Elementary School Cafeteria. A qualified preschool director or qualified staff shall always be present. Staff shall practice sanitary methods, floors shall be vacuumed and mopped daily; walls are to be washed at least weekly. Children have a fully fenced outdoor play area.

Play equipment is age appropriate. The surface of the outdoor play areas consists of wood chips, and grass. Cushioning will be provided underneath play structures to absorb falls. The outdoor play area measures for 41 capacity. Outdoor shade is provided by trees. Drinking water will be made available inside and outside the facility via a 5 gallon mobile water container and individual cups. Age appropriate play equipment are available for children's use inside the classroom. There are no bodies of water located on the property, and none are to be added without prior notification and approval of the licensing agency. The director stated no IMS will be administered at this time, but when any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. This report, as well as the AAP Guide to Safe Sleep Practices brochure, were reviewed and discussed with the applicant. All licensing reports are public information and must be made available upon request for at least three years.

Notice of Site Visit shall be posted for 30 days from today's visit.



Any proposed changes to the physical plant, telephone number, or change of address shall be immediately reported to the Department. The following needs to be completed prior to the granting of licensure. Please include facility number in all correspondence.

1. A waiver request shall be submitted and approved for a shared restroom and will require adult supervision at all times.
