32 | On 6/27/25 LPA Laird conducted five staff interviews (S1-S5). Four staff interviewed stated they were aware of an incident when a child was left on the playground without supervision. One staff stated the child was standing right behind them when the door closed momentarily. Three staff stated video footage was pulled, and they were informed by the director that the child was outside alone for 48 seconds.
On 8/14/25 and 8/18/25 LPA Laird conducted five parent interviews (P1-P5). All parents’ interviews had no knowledge of a child being left unattended on the playground. All parents interviewed stated they have never seen children left unsupervised.
On 6/27/25 and 7/14/25 LPA Laird received activity logs, incident reports and Brightwheel correspondence. Documentation received indicates C1 was left on the playground unattended for 48 seconds.
Based on the evidence obtained, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California Code of Regulations, (Title 22), is being cited on the attached LIC 9099D.
LPA Erica Laird informed facility director Heather Lingeman that this report dated 8/28/25 documents 1 Type A citation(s) which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Erica Laird informed Heather Lingeman to provide a copy of this licensing report dated 8/28/25 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
