On 10/21/21 at 9:40am, an annual required inspection was made to the facility by Licensing Program Analyst (LPA), Emilia Grisak, and met with Director Cheryl Conley. The facility file was reviewed prior to this inspection. A review of the personnel report on 10/21/21 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The facility’s operating hours are 7am - 5:30pm year round, Mon-Fri. The facility was toured at 9:40am time inside and outside and the floor and yard plan submitted by the licensee were verified.
The Director stated no firearms or weapons are stored on site and none were observed. There are no pools or similar bodies of water on the property. The items which could pose a danger to children (such as disinfectants, cleaning solutions and medications) are inaccessible to children. There were no poisons present but there is a locked closet available to store poisons if needed. Furniture and equipment are in good condition and free of hazards. There is age appropriate furniture and equipment available for infants in care. The outdoor activity space was cushioned with grass and free of hazards. There is no climbing equipment or similar equipment that would require cushioning material. Outdoor activity space is physically separate from space used by children in toddler component and preschool. Toilets and sinks are in sanitary condition and operating properly and there is a changing table available next to a sink. There is a crib available for each napping infant and cribs are kept free of all loose objects including blankets and pillows. Infants up to 12 months of age are placed on their backs for sleeping. Infants are physically checked every 15 minutes and the checks documented on facility app. Bedding is washed weekly or as needed and soiled bedding is kept out of reach of children. The facility floors were clean and safe. The kitchen/food preparation area is clean, and free of litter or rodents. Food is properly stored and free of contamination. Trash cans have tight fitting lids. Drinking water is available to children both inside and outside using water bottles and a cooler. The facility was free of flies, insects and rodents. The facility has a working carbon monoxide detector.

During today's inspection, staffing ratios were being met and there were 8 children being supervised by 3 teachers/aides. Children are not left without visual supervision at any time. The facility was operating within the licensed capacity. At least one staff member present during the visit (S1) possessed current CPR and First Aid certifications which expires on 10/2022. The sign in/out sheet was reviewed, and representatives are using full signatures and recording the time. Staff are provided on-the-job training, including sanitation and universal precautions. Children with symptoms of illness are not accepted, and children who become ill during the day are isolated in Director's office and there is a cot available for children to rest in office if needed. A weekly menu was posted in front lobby.
Five children’s records were reviewed at 10:30am, and contained emergency identification forms and medical assessments. Three staff records were reviewed at 10:15am, and contained health screening forms, proof of mandated reporter training, and proof of immunizations.
All licensing reports are public information and must be made available upon request for at least three years.

There were no Title 22 deficiencies cited during today's inspection.
