Licensing Program Analyst, Emilia Grisak conducted a case management facility inspection on 9/15/21 at 10:25am. This inspection was in response to an application for increased capacity that was received by the Department on 8/25/21. The licensees have requested a capacity increase to 14 children.

The LPA toured the facility's indoor and outdoor areas. The living room, dining room, kitchen, one bathroom, laundry room, and family room are the accessible indoor areas. The inaccessible areas include the office, 5 bedrooms including the main bedroom/bath, and an outside shed which have been made inaccessible through the use of keypad locks and baby gates. The licensees were supervising 5 children at the time of the visit, and the facility is operating within the limitations of current license ratio's. The LPA reviewed the ratio's for a large license and the licensee acknowledged understanding the ratio requirements. The LPA also discussed the Safe Sleep and Lead Exposure Testing Flyer with the licensee. There are currently no children under the age of 2 that are enrolled.



Licensee's CPR/First Aid was completed and expires on 8/8/22; Mandated Reporter training is current and licensee has taken Preventive Health Practices class on 10/17/2020. Based on the space/accommodations available at this facility and the fire marshal granting their approval on 8/30/21 for the 14 children, the capacity increase request is granted. LPA will process this capacity increase and mail an updated license to reflect this capacity change to 14 children. An exit interview was conducted with licensee.

Notice of Site Visit was given to licensee to post for 30 days.
