applicant/director stated poisons are locked in a bathroom cabinet. There is a working smoke detector, carbon monoxide detector and charged fire extinguisher rated at least 2A10BC in the facility. All Exits are marked. Items that could pose a danger to children (cleaning compounds, medication, etc) will be inaccessible. Infant children will nap in cribs. AM/PM snack and lunch will be provided by the facility and prepared in the on site kitchen. Bottles are provided by child's authorized representative and will be washed and sanitized with a 3-step process in between each use. Bottles will be labeled and stored separately. As the need changes, older infants will be provided AM/PM and lunch by the facility which will be prepared in the on site kitchen. Needs and Services plan will be updated every quarter. There is a separate kitchen for food prep, refrigeration and storage. The kitchen was inspected. Food was being properly stored and prep areas were sanitary. A sample menu is on file. The outdoor play yard measured sufficient for the requested capacity. There is safe toys and age appropriate equipment and sufficient shade. There is uncontaminated drinking water available for children both indoor and outdoor via individual water cups. There is wood chip and foam cushioning under climbing structures and swings to absorb falls. There are no bodies of water located on the property and none are to be added without prior notification and approval of the licensing agency. Unusual Incident Reporting was discussed. Children and staff records to be maintained were reviewed. Staff qualifications and ratio requirements were reviewed. Control of property will be on file. This report, along with Lead Exposure Testing Flyer, COVID protocols and the self-assessment were reviewed and discussed with the applicant/director. All licensing reports are public information and must be made available upon request for at least three years. Based on the facility measurements, capacity will be granted for 21 children.
Any proposed changes to the physical plant, telephone number, or change of address shall be immediately reported to the Department
Notice of Site Visit shall be posted for 30 days from today's visit.
The following items must be submitted prior to licensure:
1. Control of property shall be submitted.
2. Proof of enrollment for Preventative Health for both the Director and Applicant.
3. A gate shall be added to the infant nap room to separate the nap room from the play area.
A 90-day provisional license shall be granted upon submission of the above items. The provisional license cannot be extended. Prior to granting a regular license, the applicant shall provide proof of Preventative certification to complete the requirement for current Preventative Health Practices. |
