25 | On 6/29/21 at 9:00am Licensing Program Analyst (LPA) Michelle Sutton conducted a Case Management at United Methodist Preschool to operate. Director has requested to reactivate the license from inactive status. The program is located on the premises of United Methodist Church. LPA met with Director Katelyn Self and also present were a facility staff member. Director submitted and has completed the Documentation for New Directors including the CPR and First aid certification. Director stated they will be operating a summer program for preschool children (ages 3-5) using their 2 classrooms and play yard. Director stated that they will be operating a summer camp program from July 6th through July 23rd,Tuesday-Friday from 9am-12pm with an optional lunch bunch from 12pm-1pm.
At 9:05am LPA inspected classroom 1 and 2. There is a fire extinguisher near classroom 2 door (size of 3A10BC). There are working smoke and carbon monoxide detectors and a fire pull system installed near the office. Drinking water is regularly available for children. LPA observed each classroom has its own bathroom which meet the requirements of Licensing. Cleaning products and hand sanitizer are kept inaccessible to children. The facility floors of all rooms are safe and clean meeting Licensing requirements.
At 9:30am LPA inspected playground, the playground is fenced. There is ample shade with sun shade sail over a part of the playground and a large sand box that has a cover. LPA reminded director that the sand box must be inspected daily and kept free from hazardous materials and surface of the outdoor activity space must be maintained.
At 10:30am an exit interview was conducted with Director Katelyn Self, the report was discussed and signed. A notice of site visit was given and must remain posted for 30 days. License is approved to be put back on active status starting June 30, 2021.
