Licensing Program Analysts (LPAs) LaKeisha Chew and Leslie Ibo, met with Patricia Tapia, Site Supervisor for the purpose of an UNANNOUNCED CASE MANAGEMENT INSPECTION. Patricia Tapia notified the Program Director Tonya Dean on the phone to inform her we were at the facility.
The Director arrived at the facility at 3:42PM. The facility was toured with the Site Supervisor, for health and safety. Present during the inspection was eight staff and 40 children in care. LPAs initially met with Site Supervisor to notify the facility of a Case Closure for Christina Rivongkham ID # 7504134701.

YWCA of Contra Costa facility# 070207103 was sent a notice on October 04, 2019 informing the facility that this individual Christina Rivongkham did not receive a Criminal Record Clearance. The Department has ceased processing the background check for Christina Rivongkham and has closed her case. The notice stated that in order for this individual to work or be present in your facility, she must have a criminal record exemption. The notice further explained how she can request an exemption. Christina Rivongkham did not request an exemption, or she did not submit all the information within the specified time frame.
The Program Director stated Christina Rivongkham does not currently work in the facility and have never worked at this facility site. LPAs reminded the Director. Christina Rivongkham cannot be present during day-care hours. The Director is aware that criminal record exemptions are public information, as well as upon request the department will release the name of anyone with a criminal record exemption and the name of the facility where the person works or lives.

No deficiencies cited today. Exit interview conducted with Program Director Tonya Dean and she was provided a copy of their appeal rights.

Notice of site visit was posted at the time of the inspection and must remain posted for 30 days.
Failure to keep this notice posted for the 30 consecutive days will result in an immediate civil penalty.
A copy of this report shall be maintained for 3 years and available for public review upon request.
