3-LPA, Hollie met with Director, Neda Sobhiazar, for the purpose of a Random Health and Safety Inspection. There are 25 children present and nine staff present. A tour of the facility was conducted. There are no bodies of water or fire arms at the facility, per the Director. Children are being visually supervised during this visit. There are no infants being left unattended during this visit. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are inaccessible during this visit. Furniture and equipment are age appropriate and appear to be in good condition, free from sharp, loose, pointed parts or small choking articles. The surface of the outdoor activity space is free of hazards. All storage containers for solid waste, (garbage bins) have tight fitting covers that are kept on and in good repair. There is cushioning material under moveable play structures. The licensee takes measures to keep the facility free of flies, other insects and rodents. The facility has age-appropriate furniture and equipment including but not limited to cribs, cots or mats; changing tables and feeding chairs. The licensee is aware that baby walkers, bouncers, exersaucers and jumpers are not allowed in licensed care. The facility has sufficient infant napping equipment that meets Title 22 Regulation 101439.1(a)-(f). The facility has indoor space for infants that are physically separate from space used by the preschool or school age components. The child care center appears to be in good condition that ensures the safety and well-being of children, employees and visitors. LPA observed in one one a rug that is torn at the corners, exposing the backing of the carpet. The licensee states she is in the process of repairing this area of the rug. The facility will receive an advisory notice regarding the rug repair today. The facility has a functioning carbon monoxide detector. Bottles, dishes and containers of food brought by the infants authorized representative are labeled with the infants name and current date. While in use, the infant changing tables are placed within arms reach of a sink. The facility is in compliance today with the staff-infant ratio of one teacher for every four infants in attendance. The facility is aware that all person’s 18 years of age or older, must be fingerprint cleared or associated to the facility, PRIOR to being in the presence of children. During the visit today, LPA observed that the Licensing Information System, shows that a staff member's fingerprints are not cleared. LPA made contact with support staff at CCLD to inquired and was informed tjhe prints of the staff member is pending. The staff made contact with the Department of Justice and spoke with a supervisor and it was determined that the the SSN was given for the OCA number instead of the facility number as required. The supervisor, Ms. S. Combs, will e-mail the facility and update the information in the system.

The Licensee in not providing Incidental Medical Services to any infant children, per the Director.

The Director is encouraged to email childcareadvocatesprogram@dss.ca.gov to be included in the Child Care Quarterly Updates distribution list. The director is also reminded that mandated reporter training is required for all staff and is to be renewed every 2 years at www.mandatedreporterca.com.

The Licensee was made aware of Safe Sleep Regulation Concepts as it relates to Infants. During the visit, the Licensee was given a copy of the document containing the Sleep Regulation Concepts.

If the facility receives a type A violation, the violation must must be corrected by the due date. Upon receipt of a Type A violation licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. All parents/guardians must sign an acknowledgement form of proof of receiving this report (LIC 9224). The LIC 9224 must be placed in the child’s file to be reviewed by licensing.

During this visit, the facility was advised about a worn rug in the infant room that needs to be repaired or replaced as the rug is torn and worn.

As a result of this visit there are no deficiencies cited.
