Licensing Program Analyst (LPA) Paul Petersen conducted an unannounced random annual site inspection for this facility at 9:45 AM. LPA met with facility director, Farideh Bafava, and toured all areas licensed for use by children in care for a health and safety inspection. Also present were three additional teaching staff (two qualified teachers, one aide), an ABA from an outside agency and 26 preschool age children in care. The facility is within ratio and capacity and children are being supervised. All adults present are background cleared.

This preschool operates out of the entire building located at 3036 Clayton Rd, which has been divided into the main classroom, the toddler area and the play loft area. The facility has smoke detectors and working carbon monoxide detectors present and the most recent fire department inspection was in April 2021, per director. There is a fully charged 3A40BC fire extinguisher present. Per facility director, there are no firearms present or stored on the premises.

Classroom furniture and equipment are age appropriate and free of broken/sharp pieces. Surfaces including floors, counter tops and desk tops are free of hazardous conditions and toxins. Toilets and faucets are age appropriate and in good repair. There are no toxins/hazardous items observed to be accessible to children in care. The indoor loft/play structure is secure and free of broken pieces. Children's bedding for naps is stored separate per each individual child and laundered at least weekly. Sleeping mats are cleaned daily.

Children bring their own lunches and snacks daily. The facility has a supply of foods/snacks available for children as backup. LPA inspected the kitchen and food storage/food prep areas which appear to be free of pests. There is a working, age appropriate water fountain indoors as well as individual water bottles which are used for children both indoors and outdoors.

The outdoor play area is fully fenced with age appropriate play equipment. There is one piece of high climbing equipment/slides which is securely anchored and has cushioning material (sand) under and around the structure. The large structure is free of observed broken or sharp pieces. Other play structures are toddler/young child age appropriate and are free of broken/sharp pieces. Play structures are free of broken/sharp pieces or other hazards. There is shaded play area available outdoors. There are no pools or other accessible bodies of water.

A review of the facility, staff and children's records was conducted including staff education/qualifications, background clearances, staff immunization records, staff completion of mandatory reporter training, children's immunization records, parent's rights forms and admissions agreements. All staff present have current CPR/First Aid certification. Sign in, sign out records were reviewed and are accurate. There is an up to date facility roster present.

This facility provides Incidental Medical Services-IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children's, personnel, and administrative records. There were no deficiencies cited during this inspection. An exit interview was conducted with facility director. A notice of site visit was printed and is to remain posted for a period of 30 days and a copy of the appeal rights were provided. This report is to be available in the facility records for a period of three years from today's date.
