On Wednesday, April 27, 2022, Licensing Program Analyst (LPA) Caroline Colson met with Sapthala Ranasinghearach, teacher, for an unannounced complaint investigation inspection at 12:55 PM. There are nine (9) children present with two (2) staff members. A health and safety inspection was conducted. Hikkaduwa De Alwis, Center Director, arrived during the inspection. Interviews were conducted. Interviews revealed that there were a few parents who have picked up their children and their diapers needed to be changed. Based on the investigative findings, there is not enough evidence to prove that a child was left in a soilded diaper and developed a diaper rash. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation occur. Therefore, the allegation is Unsubstantiated.

On Wednesday, April 27, 2022, Licensing Program Analyst (LPA) Caroline Colson met with Sapthala Ranasinghearach, teacher, for an unannounced complaint investigation inspection at 12:55 PM. There are nine (9) children present with two (2) staff members. A health and safety inspection was conducted. Hikkaduwa De Alwis, Center Director, arrived during the inspection. Interviews were conducted. Interviews revealed that most parents didn't know if every trash bins has an actual lids or covers. Most parents were only able to come to the front door to pick up or drop off their child because of the pandemic. Based on the investigative findings, there is not enough evidence to prove that the facility didn't have lids for all trash receptacles. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation occur. Therefore, the allegation is Unsubstantiated.

On Wednesday, April 27, 2022 Licensing Program Analyst (LPA) Caroline Colson met with Sapthala Ranasinghearach, teacher, for an unannounced complaint investigation at 12:55 PM. There are nine (9) children present with two (2) staff members. A health and safety inspection was conducted. Hikkaduwa De Alwis, Center Director, arrived during the inspection. Interviews revealed that several staff members weren't wearing masks as required by the executive order before expiring on March 1, 2022. Based on LPA's interviews which were conducted and a record review, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California Code of Regulations, 101223(a)(2) is being cited on the attached LIC 9099 D.

The attached type B deficiency is being cited today and must be corrected by the due date. An exit interview was conducted. Appeal rights were given and discussed. This report must be available for public review for 3 years.
