25 | On 7/18/2022 at 11:00am Licensing Program Analyst’s Morgan Pringle and Christina Watts met with Director Kiyana Competente for an unannounced complaint investigation. Present at the facility were thirty-three (33) children and four (4) staff. During the visit LPA’s delivered findings for the complaints that were received. During LPA’s investigation it was found that the facility failed to report an incident that happened on 7/5/2022 and the Lafayette Police were called to the facility. It was also found through interview that the Director did not know the regulations for Parent’s Rights. Along with the Type B deficiencies LPA’s gave technical assistance for both deficiencies cited during the visit.
Licensee was reminded that California Law requires Licensee to report unusual incidents or injuries to children in care, to child's parents, and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). Incidents must be reported within 24 hours by phone, fax, or email. LPA informed Licensee that all forms can be downloaded at www.ccld.ca.gov.
