1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 5/3/2023 at 9:37am Licensing program analyst Morgan Pringle met with incoming Director Stefanie Galvez for a 10-day complaint visit for a complaint that was filed against the facility. During LPA's visit there were eleven (11) infants and three (3) staff present. Incoming Director was alo helping with the infants. The facility is dual licensed and holds a license for preschool (073407418). LPA toured one room (Ladybugs/Bumblebees). At 10:35am LPA conducted file reviews and it was found that one (1) staff (S5) member did not have a cleared criminal record clearance. LPA Pringle contacted the Oakland South East Child Care office for verification and it was confirmed that the staff member had not yet obtained the full clearance. LPA informed incoming Director that S5 must not be present at the facility until a criminal record clearance is obtained. During the complaint visit it was found that three (3) teachers present did not have the proper qualifications and all four (4) staff working with the infants did not have the required three (3) infant toddler units, which places the facility out of ratio. Deficiencies Cited S5 did not obtain a criminal record clearance Facility out of ratio due to unqualified staff LPA Pringle informed incoming Director that this report dated 5/3/2023 document(s) 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Pringle informed the Director to provide a copy of this licensing report dated 5/3/2023 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

An immediate $500 civil penalty is assessed today and $100 per day will be assessed until corrected.

A notice of site visit was given to licensee and must be posted for 30 days.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
