On 6/23/2023 at 8:50am, Licensing Program Analyst (LPA) Julia Placencia arrived at the facility unannounced for a Required 1-Year Inspection. LPA met with director Humaira Ahmed and licensee Rozita Dadgostari. There were 11 infants and an additional 3 staff members present. This program is part of a combination center that also includes a preschool component. The facility was toured for a health and safety inspection. The hours of operation are Monday through Friday, 7:30am to 6:00pm.

Furniture and napping equipment observed during inspection were age appropriate and in good condition. The indoor and outdoor activity space for the infant program is physically separate from the preschool component. Each crib is occupied by only one infant at a time, and is free from all loose objects, including blankets and pillows. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children were inaccessible during the inspection. The floors were free of tripping hazards. Kitchen/food preparation and storage areas were observed to be clean, and free of evidence of rodents, and food is protected against contamination. All storage containers for solid waste have tight-fitting covers that are in good repair. Drinking water is available both indoors and outdoors. Menus were posted and visible for parents to view. There are no pools or similar bodies of water at this facility. Outdoor play equipment was observed to be safe and free of hazards. There is a shaded area provided for the children. Required licensing documents were posted. Fire/Disaster Drills are conducted monthly, and a review of the log shows the last drill was conducted 6/19/2023. Fire extinguishers, carbon monoxide and smoke detectors meet State Fire Marshall standards.

The facility is operating within its licensed capacity. The facility is within ratio today with 1 teacher supervising no more than 4 Infants. LPA did not observe any child left without visual supervision during the inspection. LPA verified both opening and closing staff have current CPR/First aid training. The center is in compliance with the sign in and out procedure.

At 10:55am, staff files and a random selection of children's files were reviewed and contain required licensing documents. Infants up to 12 months have Individual Infant Sleeping Plans, and the facility maintains a sleep log for infants up to 24 months. Infant fiiles reviewed also contain updated needs and services plans. Staff have mandated reporter training, and required immunizations. Teacher present today meet qualification requirements.

Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.


Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Licensee was encouraged to frequently visit our website at www.ccld.ca.gov for licensing regulations and updates, and to also email childcareadvocatesprogram@dss.ca.gov to be included in the Child Care Quarterly Updates distribution list.



LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. ***Continued on LIC 809C....

The following deficiencies were cited during today's inspection:
  • At 9:45am, LPA found that infant bedding was not being changed daily.
  • At 10:00am, LPA observed several holes in the fence slats in the infant/toddler outdoor play yard which a child can reach his hand through.

See LIC809D for deficiencies cited during today's inspection. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.
