Licensing Program Analyst (LPA) Redmond, conducted an announced, Complaint Investigation follow up visit from an 05/09/2019 visit. LPA met with JESSICA BARRAZA, DIRECTOR AND SANGITA HINGORANI, OWNER. During the previous visit of 05/09/2019 and today's visit, LPA remained at the facility and made personal observations in each of the classrooms. Additionally, LPA conducted interviews of staff. Based on LPA's observations and interviews, LPA has made the following determinations:

Allegation: Lack of supervision resulting in children playing in the bathroom: LPA observed a fully qualified teacher take eight (8) children to the restrooms. There are two separate restrooms, side by side. As each child used the restroom, some children waited in line. While the teacher focused on children in one stall, LPA observed a child sitting on the toilet while four other children (boys and girls) came into the restroom while the child was on the toilet. The teacher did not notice. Additionally, while the teacher tended to the children in the restroom, LPA observed a child turning the handle of a door. The teacher did not see the child. Based on LPA observations, the preponderance of evidence standard has been met. Therefore,
