An announced Prelicensing Visit was conducted today, October 6, 2021 at 10:00am by Licensing Program Analyst (LPA) Melissa Guirit. LPA met with Applicants, R. La Field, A. Maggio, and M. Maggio and Directors, R. Dubois and J. Heitman. The applicant has submitted an application for a change of ownership. The facility is located onsite a church. The facility will operate in four rooms. Two rooms are on the lower level and two rooms on the upper level. The first room on the upper level to the left has a kitchen for snack preparation. The program will operate from 7:30 AM - 6:00 PM. LPA toured the facility to conduct a health and safety inspection inside and outside. There are no structural changes to the facility and measurements remain the same. The center has obtained an approved fire inspection clearance on 7/15/2021.

The preschool rooms are equipped with varied age appropriate materials and equipment. There are two bathrooms for children use. One bathroom is located inside the first classroom located on the lower level to the right. The other bathroom is located on the upper level, where the teachers will assist and supervise the children to use the bathroom at all times. The isolation area will be in the staff office.

There are two play yards separated by fencing. The small climbing structure on the left play yard for the younger children is sufficiently cushioned. There is sufficient shade provided in the play yard. The center has water available to children for indoor and outdoor activities. Am/pm snacks are being provided by the facility. Families will be required to provide their own lunches for children. No cleaning agents were found stored with food. First aid supplies are available in the center. There is a working telephone on site. Fire alarm/Carbon monoxide is available. Facility is equipped with a fully charged 3A40BC fire extinguisher. The sign in/out sheets provide for a full signature.

If this facility plans to provide Incidental Medical Services – IMS. A Plan of Operation that includes IMS must be submitted to the Department. Mandated reporter and appeal rights, civil penalties, unusual incident reporting and fingerprint requirements were discussed today. LPA will email COVID-19 Self-Assessment Guide and Applicant submit to LPA once completed. LPAs reviewed responses and provided technical assistance including postings. Licensee is also being informed of the web address (www.ccld.ca.gov) for downloading child care forms, and the director is encouraged to email ChildCareAdvocatesprogram@dss.ca.gov to be included in the Child Care Quarterly Updates distribution list. The director is also reminded that mandated reporter training is required for all staff and is to be renewed every 2 years at www.mandatedreporterca.com. Zero Tolerance policies were explained. The center was found to be clean, safe, sanitary, and in good repair.

A license for 68 children will be issued effective today 10/6/21.

A copy of this report was emailed to Director due to printer issues, appeal rights, and notice of site visit were provided to Director, J. Heitman. Exit interview conducted with Director.
