**VISIT CONDUCTED VIA ZOOM PLATFORM DUE TO COVID-19 PANDEMIC**

A Prelicensing Visit was conducted today VIA ZOOM by Licensing Program Analyst (LPA) Melanie Otsuji. LPA met with applicant Rachel Liles. The applicant has submitted an application for change of ownership for an Infant License. The center will operate Monday thru Friday 6:30am-6:30pm. The infant program will utilize Suite A4, which includes an eating area, play/activity area, and nap room, the women's bathroom in the hallway, and the outdoor play area. LPA virtually toured these areas to conduct a health and safety inspection, and the measurements previously on file are as follows:

INDOORS: 606.7 square feet = 17 children
OUTDOORS: 681.5 square feet= 9 children
A fire clearance for a total capacity of 12 infants was received from the San Ramon Valley Fire Protection Department on 9/11/2020.

The center has sufficient heating, lighting and ventilation for the safety and comfort of children and staff. The classroom floors and surfaces are clean and in good repair, and there is an ample supply of age appropriate toys, activities, equipment and storage available for children. The center is equipped with a centralized combination smoke/carbon monoxide detector, working telephone, and first aid kit. The food preparation area is clean and has sufficient equipment and storage space. LPA did not observe any cleaning supplies stored where food will be stored. The center will supply snacks, and families will bring meals. LPA did not observe any bodies of water, hazardous items or toxins accessible today. The changing table is within arms reach of a functioning sink. The center will have children utilize water bottles both indoors and outdoors. The sinks are in sanitary and operational condition, and there will be potty seats available for potty training. The isolation area will be the space at the entrance of the infant room, separated from other children, and the staff bathroom stall.

The playground is fully fenced, and the building has an overhang which provides shade. The nap room has space for 5 cribs. LPA observed mats available for older children, and they were stored appropriately.

LPA has reviewed documents and verified that Rachel Liles is qualified as an infant director.

LPA reminded the director that the mandated reporter training needs to be completed by all staff, and renewed every 2 years at www.mandatedreporterca.com. The director was reminded that commingling between programs is not allowed, and each program will need to maintain separate spaces and staff. LPA reminded the director that staff need to have a fingerprint clearance and be associated to this facility, prior to employment and working with children, or a civil penalty can be accessed. LPA reviewed the reporting requirements for injuries requiring medical attention and unusual incidents. LPA reminded the director to ensure that whole milk is served to children 2 years and under.


The center is recommended for a provisional license for 12 infants, including 5 crib age children effective as of today's date 10/28/2020. A standard license will be issued once requested documents are submitted.

This report will remain on file for 3 years. An exit interview was conducted.
