13 | Licensing Program Analyst (LPA) Michelle Pascual met with Site Director Paris Schmidt via a telephone call to deliver findings for the above allegations. During the investigation, LPA Pascual conducted interviews with children, parents and staff to gather information about the allegations. The allegations stated that a child sustained unexplained injuries while in care and facility staff did not assist child with toileting needs.
During the course of the interviews no evidence was found to support that a child did receive unexplained bruises while in care nor was there evidence that staff do not help with toileting needs. LPA did learn that all injuries were reported via an “ouch” report and sent home with parents. LPA witnessed these cards in the files of the children as well. When LPA discussed toileting needs, it was found out that the children are reminded to utilize the bathroom every 45 to 60 minutes when in transition and for those that are in diapers, they are changed every couple of hours.
LPA also witnessed adequate supervision amongst the children and that staff promptly intervenes if there are behavioral issues.
Based on the information received and witnessed, the above allegations could not be substantiated nor dismissed. Although allegations may have taken place there is not enough evidence to support them at this time. Therefore the above allegations are deemed UNSUBSTANTIATED.
An exit interview was conducted with the Site Director and appeal rights were provided.
