On 07/19/2022, Licensing Program Analyst (LPA) Arianna Manabat and Licensing Program Manager (LPM) Natalie Dunaway, met with the director, Lisa J. Hester, to deliver findings for the complaint alleging staff member handled day care child in inappropriate manner during the nap time.

During the investigation, LPAs interviewed the staff, random children, random parents and reviewed the records. Due to conflicting evidence, interviews conducted do not support the allegation that a staff member handled child in rough manner. The preponderance of evidence has not been met, therefore, the above allegation is found to be UNSUBSTANTIATED. Exit interview was conducted and a Notice of site visit was issued. Notice of site visit is to be posted and shall remain posted for next 30 days.

The director agreed to conduct a training with all staff on safe children handling procedures and director will submit the attendance sheet of staff members.

 interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. This requirement is not met as evidenced by during the interviews, it was found that there have been times, when children got injured at the facility. This poses an immediate health and safety risk to children in care.

A Type “A” violation (see continuation) was issued today. The center is informed to provide a copy of the Evaluation Report and the Type “A” Deficiency cited to the parents and guardians of children currently enrolled in care and to parents of newly enrolled children during the next 12 months. A signed and dated LIC 9224 shall be maintained in all Children's files. This report and appeal rights were provided and reviewed with the licensee. Notice of Site Visit shall remain posted for 30 days.
