LPA Manabat investigated a diaper rash incident pertaining to c1 as the rash had formed over the course of a few hours. LPA Manabat obtained record review indicating that diaper cream was not provided by the authorized representative and that there were attempts made to obtain the cream.
Finally, LPA Manabat investigated the child falling due to a lack of supervision. Per interviews with the staff members and the Reporting party, the child fell backwards while sitting up. The child was asked by their authorized representative to sit up and, as a result, fell backwards onto a cushioned mat and did not sustain injury such as bruising, scratches, swelling, or redness to their body. The infant was under direct supervision by a teacher at the time of the incident and was immediately assessed for any injury.
Based on the information obtained, LPA was not able to determine that an infant fell while in care due to lack of supervision, staff are not labeling infants bottles, and staff are not providing adequate hygiene needs to infants in care. Therefore, the allegation is determined to be UNSUBSTANTIATED, meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove it. An exit interview was conducted.
Notice of site visit and appeal rights were also addressed. The Notice of site visit shall remain posted for 30 days for guardian/parental review. |
