Application Specialist (AS) Mai Lor met with Applicant Leslie Amato for the purpose of an announced change of location prelicensing inspection. Applicant is currently licensed under facility number 093610482. Applicant requests a preschool license to serve children from age two to entry into kindergarten. The facility operates on Lake Tahoe Community College Campus in the Early Learning Center building. The facility is a Parent Co-Op program. The program will operate Monday through Thursday from 9:00 AM to 12:00 PM. The fire clearance was granted on 9/21/2021.

Applicant acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, menus, and daily schedule. AS discussed the forms that must be in each child's and each staff member's file. The facility will be providing morning snacks.

INDOOR ACTIVITY SPACE:
There is one preschool classroom that is split into two classrooms, a mud room, a cooking activity space, and a reading nook available to children. AS observed sufficient amount of equipment, toys, tables, and chairs to accommodate the requested capacity of 40 preschool children. The first aid kit, medication box, and cleaning disinfectants are inaccessible to children. Applicant stated there are no poisons or firearms on the premises. Applicant stated in addition to the water fountains, a water cooler or pitcher and cups will be available in all classrooms. AS observed functional carbon monoxide detectors in each of the classrooms. AS observed a paper sign-in/sign-out system.

AS measured the classroom and activity space. The total classroom and activity space contains a total of 1524.38 square feet, which will accommodate Applicant's request for 40 children. There are three toilets and six sinks for the children, and a separate private restroom for the staff. Individual measurements are recorded on the Capacity Worksheet (LIC 9024).

Children who become ill during the day will be isolated in the reading nook area and will use the staff restroom, if necessary.

OUTDOOR ACTIVITY SPACE:
There is one outdoor area on the property. This outdoor space is under construction. Per Applicant and Project Manager Al Frangione, they are projecting the outdoor play yard to be completed by December 2021. The play area is fenced with chained link fence that is at least four feet tall. Applicant was reminded to contact licensing upon completion of the playground for a follow-up inspection. Applicant submitted a shared outdoor activity space with Lake Tahoe Community College Child Development . AS observed a sufficient amount of equipment and toys. There are no bodies of water on the premises. There is shade supplied by trees and overhangs.

AS measured the outdoor space. The outdoor play area contains a total of 12,032.18 square feet, which will accommodate Applicant's requested capacity of 40 children. Individual measurements are recorded on the Capacity Worksheet (LIC 9024).

AS was provided with updated Board Resolution, Administrative Organization (LIC309) and proof of control of property.

This facility plans to provide Incidental Medical Services - IMS. For IMS information, see Evaluator Manuel - Regulation Interpretations and Procedures for Child Care Centers Section 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. AS discussed the following: 100% supervision is required at all times, including in the bathroom; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. AS discussed with Applicant any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days. Prior to this inspection, AS emailed LIC311A, Effects of Lead Exposure brochure and Provider Notice Information (PIN) 21-21 (Release of the Written Directives for Lead Testing of Water in Licensed Child Care Centers per AB 2370), and immunization card. AS reviewed lead testing requirement and COVID-19 technical assistance with the applicant.

Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Licensee was encouraged to visit the Department website at WWW.CCLD.CA.GOV for child care updates, current forms, legislation and regulation information. A copy of this report will remain on file for a period of three years for public review upon request.

Effective today, 9/24/2021, AS will approve the facility for a capacity of 40 preschool children licensed to serve children age 2 to entry into kindergarten.
